痔血合剂改善溃疡性结肠炎患者肠道出血的回顾性研究

Retrospective Study of Zhixue Mixture in Improving Intestinal Bleeding in Patients with Ulcerative Colitis

目的观察痔血合剂在活动期溃疡性结肠炎改善肠道出血缓解中的临床疗效并探讨其作用机制。方法将80例溃疡性结肠炎伴有便血的患者运用随机数字表法分为两组,对照组予常规标准化治疗,治疗组在上述基础上用痔血合剂口服。观察患者临床症状,结肠镜病理情况,便血改善程度以及血红蛋白(Hb)、红细胞计数(RBC)、血小板计数(PLT)、凝血酶原时间(PT)、活化部分凝血酶时间(APTT)、血浆凝血酶时间(TT)及血浆纤维蛋白原(FIB)等指标。结果治疗组显效率为92.50%,高于对照组的40.00%(P<0.05);两组病例治疗后便血改善程度评分优于治疗前(P<0.05),且治疗组优于对照组(P<0.05);两组病例治疗后RBC、Hb、PLT高于治疗前(P<0.05),且治疗组高于对照组(P<0.05);两组病例治疗后APTT、TT低于治疗前(P<0.05),FIB高于治疗前(P<0.05),且治疗组优于对照组(P<0.05)。PT两组治疗前后差异无统计学意义(P>0.05)。结论对于溃疡性结肠炎肠道出血的患者,在常规治疗的基础上,辅以痔血合剂治疗,可以改善患者血清学指标,早期缓解出血症状,提高生活质量。

Objective： To explore the clinical curative effecacy of Zhixue Mixture on intestinal bleeding in active ulcerative colitis,and to discuss the mechanism of its action. Methods： 80 patients with ulcerative colitis and hematochezia were randomly divided into the treatment group and the control group,with 40 cases in each group.The two groups were treated with routine standardized treatment,and the treatment group was added oral Zhixue Mixture on the basis. The clinical symptoms,pathological condition of colonoscopy,hematochezia improvement and RBC,Hb,PLT,PT,APTT,TT and FIB were observed. Results： The effective rate of the treatment group was92.50%,higher than that of the control group 40.00%（P〈0.05）. After treatment,the improvement rate of hematochezia in the two groups was better than that before treatment（P〈0.05）,and the treatment group was better than the control group（P〈0.05）. After treatment,RBC,Hb and PLT of the two groups were higher than those before treatment（P〈0.05）,and the treatment group was higher than the control group（P〈0.05）. After treatment,the APTT and TT of the two groups were lower than those before treatment（P〈0.05）;FIB was higher than that before treatment（P〈0.05）,and the treatment group was better than the control group（P〈0.05）. There was no significant difference in PT between the two groups before and after treatment（P〈0.05）. Conclusion： On the basis of routine treatment,with the treatment of Zhixue Mixture,the patients with ulcerative colitis can improve the serological index,relieve the bleeding symptoms early and improve the quality of life.
