舍曲林片合并结构式家族治疗老年抑郁症患者睡眠质量的临床观察

Clinical Observation of Sertraline Combined with Structural Family Therapy in the Sleep Quality of Senile Depression

目的：探讨舍曲林片合并结构式家庭治疗对老年抑郁症睡眠质量的影响。方法：将136例老年抑郁症患者,按单双号随机分为观察组和对照组各68例,两组患者均予舍曲林片治疗,观察组同时联合结构式家庭治疗,疗程均为8周。两组患者在治疗前、治疗4周和治疗8周后进行汉密尔顿抑郁量表（HAMD-17）和匹兹堡睡眠质量指数问卷（PSQI）调查。结果：治疗4周后,观察组和对照组的有效率分别为76.47%和73.53%,差异无统计学意义（χ^2=0.157,P=0.692）,但8周后,两组的有效率比较,差异有统计学意义（χ~2=5.445,P=0.02）;比较两组患者治疗前HAMD-17和PSQI评分,差异均无统计学意义（t=1.75,1.191;P〉0.05）;观察组患者治疗4周后HAMD-17和PSQI评分均比对照组明显降低（t=5.735,1.996;P〈0.05）,8周后观察组患者的两项评分下降更为明显（t=3.352,6.787;P〈0.01）。结论：舍曲林片联合结构式家庭治疗老年抑郁症,可以有效降低患者的抑郁情绪,改善患者睡眠质量,提高生活质量。

Objective：To observe the clinical efficacy of sertraline combined with structural family treatment of senile depression.Methods：A total of 136 cases of senile depression were randomly divided into observation group and control group.All patients were given the sertraline.On the basis of this treatment,the patients of observation group were treated with combined structural family therapy.The course of treatment was eight weeks.The Hamilton Depression Scale（HAMD-17）score,Pittsburgh Sleep Quality Index table（PSQI）score were assessed.Results：After 4 weeks,the clinical effective rates in observation group and control group were 76.47% and 73.53%,with no significant differences（P〉0.05）,but they were significant differences at 8 th week（χ^2=5.445,P〈0.05）.There was no significant difference in HAMD-17 and PSQI score between the two groups before the treatment（P〉0.05）;after four weeks,HAMD-17 and PSQI score of observation group were significantly lower than those in the control group（t=5.735,1.996;P〈0.05）.The HAMD-17 and PSQI score in the observation group were more lower than those in the control group（t=3.352,6.787;P〈0.01）at the 8 th week.Conclusion：Sertraline combined with structural family treatment treating senile depression can effectively eliminate or relieve patients＇ clinical symptoms,improve the sleep quality and their life quality.
