丙戊酸钠合并碳酸锂与阿立哌唑联合氯氮平对双相Ⅰ型躁狂IL-17表达的影响及疗效

Comparison of the Effects of Sodium Valproate Combined with Lithium Carbonate and Aripiprazole Combined with Clozapine on the Expression of Biphasic Type Ⅰ Mania IL-17 and Their Therapeutic Effect

目的：研究分析丙戊酸钠合并碳酸锂与阿立哌唑联合氯氮平对双相Ⅰ型躁狂白介素17（interleukin 17,IL-17）表达的影响及临床治疗效果。方法：2015年9月-2016年9月期间收治的双相Ⅰ型躁狂患者72例,按照随机、对照、双盲试验原则分为观察组和对照组各36例,对照组采用阿立哌唑联合氯氮平治疗,观察组患者采用丙戊酸钠合并碳酸锂治疗。结果：治疗1周、4周、6周,观察组患者BRMS评分显著低于对照组（t=-10.458,-26.019,-18.573;P〈0.01）,观察组患者IL-17水平明显低于对照组（t=-8.213,-16.795,-25.483;P〈0.01）;观察组患者治疗总有效率88.89%,明显高于对照组69.44%,差异具有统计学意义（χ^2=4.126,P〈0.05）。结论：对双相Ⅰ型躁狂患者,丙戊酸钠合并碳酸锂治疗效果优于阿立哌唑联合氯氮平,有利于减轻躁狂症状,改善炎症反应,提高临床治疗效果。

Objective：To study and analyze the effect of sodium valproate combined with lithium carbonate and aripiprazole combined with clozapine on the expression of interleukin 17（IL-17）and the clinical therapeutic effect.Methods：Seventy-two patients with bipolar disorder enrolled from September 2015 and September 2016 were selected and divided into the observation group（n=36）and the control group（n=36）according to randomized,controlled and double-blind trial principles.Patients in the control group was treated with aripiprazole and clozapine,while patients in the observation group were treated with sodium valproate combined with lithium carbonate.Results：After 1 weeks,4 weeks and 6 weeks,the BRMS score of observation group was significantly lower than that of the control group（t=-10.458,-26.019,-18.573;P〈0.01）,and the level of IL-17 in the observation group was significantly lower than that of the control group（t=-8.213,-16.795,-25.483;P 0.01）;The total effective rate of the treatment group was 88.89%,which was significantly higher than that of the control group（69.44%）（χ^2=4.126,P〈0.05）.Conclusion：The treatment effect of sodium valproate combined with lithium carbonate is better than that of aripiprazole combined with clozapine,which is helpful to reduce the mania symptoms,improve the inflammatory reaction and the clinical therapeutic effect.
