小切口非超声乳化囊外摘除术对老年白内障患者眼压及视力恢复的影响

Effects of small incision non phacoemulsification extracapsular cataract extraction on intraocular pressure and visual acuity in senile patients with cataract

目的分析小切口非超声乳化囊外摘除术对老年白内障患者眼压及视力恢复的影响。方法选取老年白内障患者70例,将所有患者随机分为两组,每组35例。对照组患者采用超声乳化白内障吸除术,观察组患者采用小切口非超声乳化囊外摘除术,比较两组手术前后眼压、视力变化及不良反应发生率。结果术前,两组患者眼压及视力变化比较,差异未见统计学意义(P>0.05);术后2、6、12、24 h,观察组患者眼压低于对照组,差异有统计学意义(P<0.05);术后48 h,两组患者眼压比较差异未见统计学意义(P>0.05);术后1周,观察组视力水平高于对照组,差异有统计学意义(P<0.05),术后3个月,两组视力水平比较,差异未见统计学意义(P>0.05);观察组不良反应发生率(14.29%)低于对照组(34.29%),差异有统计学意义(P<0.05)。结论小切口非超声乳化囊外摘除术可有效恢复老年白内障患者的视力,控制眼压升高,不良反应较少,治疗效果显著,在临床应用中值得推广。

Objective To analyze the effects of small incision non-phacoemulsification extracapsular cataract extraction （ECCE） on intraocular pressure and visual acuity in patients with cataract. Methods A total of 70 senile patients with cataract were selected and randomly divided into two groups, with 35 cases in each group. The patients in the control group were treated by phacoemulsification, and the patients in the observation group were treated by small incision non phacoemulsification extracapsular cataract extraction, and the intraocular pressure, visual acuity and adverse reactions were compared between the two groups before and after operation. Results Before operation, there was no significant difference in the intraocular pressure or visual acuity between two groups （P 〉 0. 05 ） ; at 2, 6, 12 and 24 h after the operation, the intraocular pressure of the observation group was lower than those of the control group, the differences were significant （P 〈 0. 05 ） ; at 48 h after the operation, there was no significant difference in intraocular pressure between the two groups （ P 〉 0. 05 ） ; at 1 week after the operation, the visual acuity in the observation group was significantly higher than that in the control group, the difference was significant （P 〈 0.05 ） ; at 3 months after the operation, there was no significant difference in visual acuity levels between the two groups （ P 〉 0. 05 ） ; the incidence of adverse reactions in the observation group （14.29%） was lower than that in the control group （34. 29% ）, and the difference was significant （P 〈 0. 05 ）. Conclusions The small incision non phacoemulsification extracapsular extirpa- tion can effectively restore the visual acuity of the senile patients with cataract, control the increase of in- traocular pressure, has less adverse reactions and good treatment effect, which is worthy of clinical application.
