临床药师实时监测细菌耐药情况分析与体会

摘要 目的实时监测细菌耐药情况,协助临床选择更适宜的抗菌药物,防止细菌耐药。方法临床药师每日实时监测微生物实验室细菌培养结果,重点关注药敏结果与所用药物不适宜的患者,对多重耐药菌尤其重点关注,科室临床药师随时监测抗菌药物使用后续情况,发现问题及时反馈医师,共同商讨治疗方案。结果微生物实验室细菌培养阳性结果 479份中,临床药师通过实时监测细菌耐药,共干预不适宜的抗菌药物48例,干预率为10%,尤其多重耐药菌药物的选择,不仅防止细菌进一步耐药,促进疾病治愈,同时也提升了临床药学工作开展的深入性。结论在细菌耐药监测工作中,临床药师不能仅限于定期分析抗菌药物对细菌的耐药率,更多的是通过实时监测细菌耐药,提前干预不适宜的抗菌药物,帮助临床解决实际问题,确实做到防止细菌耐药。 Objective Monitor bacterial drug resistance in real time,help to select suitable anti-bacterial clinical medicine,and prevent bacterial drug resistance.Methods The clinical pharmacist monitored the lab cultivation of microorganism bacteria in real time daily,specially paied attention to the patients who had drug allergy and for whom the drugs administered were not suitable,and observed closely the bacteria resisting multiple drugs.The clinical pharmacist monitored the subsequent situation anytime after the anti-bacterial drug was used,and feared back to the doctors of any issues identified for discussion on treatment plan.Results Among the 479 positive cases of the microorganism lab bacteria cultivation,48 cases of unsuitable antibacterial drugs were intervened,intervened rate was 10%.The selection of drugs with multiple drugresistant bacteria can not only prevent the further bacterial drug resistance,promoted the cure of diseases,and also enhanced the depth of clinical pharmacy work.Conclusion When monitoring bacterial drug resistance,pharmacists shall not limit their work to periodically analyze drug resistance ratio of antibacterial drug to baceteria.More importantly,they can intervene with the unsuitable antibacterial drugs through real time bacterial drug resistance monitoring,so as to help resolving actual clinical problems and preventing bacterial drug resistance.
