从郁论治类风湿性关节炎思路浅析

Treatment of Rheumatoid Arthritis with the Perspective of Depression

[目的]探究从郁论治类风湿性关节炎的中医辨治思路。[方法]参考中医古籍及当代医家的相关论述,结合笔者临床实际病例,从郁之为意、郁证征象、因郁致病、从郁论治及病案解析等五方面分别进行论述,探讨从郁论治类风湿性关节炎的中医辨治思路及临床实际效果。[结果]郁可分为气机郁滞和情志抑郁,两者均可导致湿聚血瘀、脏腑失调而致病。中医古籍及当代医家的相关论述及笔者的临床实际病例均显示,类风湿性关节炎的诸多症状表现均有郁证征象。郁证在类风湿性关节炎上的征象,包括关节症状、全身症状和干燥综合征表现。从郁论治类风湿性关节炎,治疗上以行气开郁为主,辅以其他治法,可取得良好临床疗效。[结论]郁是引起并加重类风湿性关节炎的一个重要因素,从郁论治类风湿性关节炎,有充分的中医理论依据和较好的临床疗效,值得进一步研究与探讨。

[Objective] To treat rheumatoid arthritis from the perspective of depression in the setting of traditional Chinese medicine. [Methods] Reference to ancient Chinese medical books and contemporary medical experts,we summarized the treatment of rheumatoid arthritis via the perspective of depression from what＇s the meaning of depression, signs of depression syndrome, depression causing disease, theory of governance from depression and these aspects and analysis of medical case records. [Results] Stagnation of the Qi and emotional depression both can cause dampness and blood stasis and internal organs disorders. Depression signs can be found in rheumatoid arthritis,including joint symptoms, systemic symptoms and Sjogren＇s syndrome. Promoting the circulation of Qi is very important in treatment of rheumatoid arthritis from the perspective of depression. [Conclusion] Depression is an important factor causing and aggravating rheumatoid arthritis. There is a full theoretical basis for traditional Chinese medicine and good clinical efficacy in treatment of rheumatoid arthritis from the perspective of depression.
