延伸综合康复护理干预脑卒中后吞咽障碍患者的效果评价

Extended Comprehensive Rehabilitation Nursing for Patients with Dysphagia after Stroke

目的:探讨延伸综合康复护理干预脑卒中后吞咽障碍患者的效果。方法:将70例脑卒中后吞咽障碍患者随机分为观察组(35例)和对照组(35例),对照组给予常规治疗和护理,观察组则在常规治疗和护理后回到病房进行摄食训练,分别于治疗前、治疗1个月后利用标准吞咽功能评价量表(SSA)进行吞咽功能评估,吞咽障碍生活质量量表(SWAL-QOL)评价其生存质量,治疗1个月后进行吸入性肺炎检查。结果:观察组SSA、SWAL-QOL评分改善程度显著优于对照组(P<0.05);两组吸入性肺炎发生率差异无统计学意义(P>0.05)。结论:延伸综合康复护理干预有助于脑卒中后吞咽障碍患者的吞咽功能及生活质量的改善,但对降低吸入性肺炎发生率作用不明显。

Objective： To investigate the effect of extended comprehensive rehabilitation nursing for patients with dysphagia after stroke. Methods： 70 cases of patients with dysphagia after stroke were randomly divided into observation group（35 cases） and control group（35 cases）, the control group accepted conventional treatment and nursing while observation group received feeding training when returned to the ward after conventional interventions,then used Standardized Swallowing Assessment（SSA） to evaluate their swallowing function and SWAL-QOL for quality of life at the time of before and 1 month after treatment, and detected the occurrence of aspiration pneumonia after 1 month of treatment. Results： The improvement of SSA and SWAL-QOL scores of observation group were significantly better than those of control group（P〈0.05）; there was no statistical difference between the incidence of aspiration pneumonia in the two groups（P〉0.05）. Conclusion： Extended comprehensive rehabilitation nursing is helpful to the improvement of swallowing function and life quality of patients with dysphagia after stroke, but can＇t help reduce the incidence of aspiration pneumonia.
