难治性癫痫患者立体定向脑电图视频脑电图监测的护理干预

Nursing intervention for patients with intractable epilepsy undergoing stereo - electroencephalography monitoring

目的总结立体定向脑电图（SEEG）电极置入患者视频脑电图监测的护理要点。方法回顾100例在机械臂辅助手术系统辅助下置人立体定向电极患者的临床资料，通过对电极置人术后护理过程特殊问题的分析，总结专科护理经验。结果100例患者中5例（5％）出现并发症，其中2例出现一根电极松动，3例术后出现少量出血，其余95例（95％）患者未出现电极移位、颅内感染、出血等临床意外事件，均顺利完成SEEG监测，并经多学科讨论根据手术计划成功进行了致痫灶切除手术；平均监测时程14．2天。结论在视频脑电图监测期间，对颅内电极置入癫痫患者实施相关特殊护理干预，不但可预防立体定向电极置人并发症的发生，保证监测质量，也为精确的癫痫定位诊断、外科手术和术后康复提供安全保障。

Objective To summarize the nursing key points of stereo - electroencephalography （SEEG） monitoring in patients accepted stereotactic-electrode implantation. Methods The clinical data of 100 patients after stereotactic-electrode implantation by the robot-arm assisted system were reviewed. By the analysis of problems during the nursing process after stereotactic-electrode implantation, the experience of specialized nursing was summarized. Results Among the 100 patients, only 5 patients had complications, including 2 patients with one electrode loosing and 3 patients with intracranial hemorrhage. The other 95 patients were safely during SEEG monitoring and had no clinical accidents such as electrode shifting, intracranial infection and hemorrhage, with the average monitoring time of 14.2 days. After a multidisciplinary discussion, the tailor-resection of the epileptic zone was successfully performed. Conclusions During SEEG monitoring, special nursing interventions were performed on epilepsy patients after stereotactic-electrode implantation, which could not only prevent the complications and ensure the safety of monitoring, but also helped to find the accurate epileptic zone for tailor resection and ensure the postoperative rehabilitation.
