血清降钙素原在终末期肾病合并细菌性脓毒症患者中的诊断价值 被引量:6

摘要 目的探讨血清降钙素原(PCT)在终末期肾病合并细菌性脓毒症患者中的诊断价值。方法回顾性分析符合2012年国际肾脏病组织制定的KDIGO终末期肾病诊断标准的83例患者的临床病例资料。根据ACCP/SCCM脓毒症诊断标准,将其分为脓毒症组(n=38)和非脓毒症组(n=45),比较两组中的PCT水平,应用受试者工作特征(ROC)曲线评价其对终末期肾病患者的诊断价值。结果脓毒症组的PCT水平显著高于非脓毒症组(P<0.01)。脓毒症组中的PCT水平为(11.38±8.29)ng/m L,明显高于PCT临床应用专家共识中普通脓毒症患者的PCT诊断界值(>0.05 ng/m L)。通过ROC曲线分析得出:PCT用于诊断终末期肾病合并细菌性脓毒症患者的最佳截点值为2.665 ng/m L,其敏感度为88.9%,特异度为100.0%。结论PCT是诊断终末期肾病合并细菌性脓毒症患者的良好标志物,当PCT水平>2.665 ng/m L时能更有效地诊断脓毒症。
