丹芷祛痘中药外敷治疗青少年肺胃湿热型痤疮临床随机对照研究

External Application of Dan-Zhi Herbal Compounds Treat Acne Vulgaris in Adolescents Diagnosed as Syndrome of Dampheat Stagnation in Lung and Stomach Randomized Controlled Trial

目的:观察丹芷祛痘中药外敷对青少年肺胃湿热型痤疮的临床治疗效果。方法:按照完全随机原则把134例1~3级青少年肺胃湿热型痤疮病例分为实验组与对照组,实验组病例68例,予丹芷祛痘中药贴敷患处;对照组病例66例,予维A酸乳膏和红霉素醋酸锌凝胶外擦患处。治疗2个疗程后进行临床疗效评价,一个疗程为3 w,研究组成员在入组病例完成2个疗程的4 w后进行随访。结果:实验组与对照组病例在2个疗程后痤疮患处皮损变化差异有统计学意义(P<0.05),实验组病例的总有效率为92.05%(64/68),超过对照组病例的总有效率71.21%(46/66),2组间差异有统计学意义(P<0.05);实验组治疗后随访复发率15.63%(10/64),对照组治疗后随访复发率36.96%(17/46),2组间差异无统计学意义(P>0.05)。结论:丹芷祛痘中药外敷对治疗青少年肺胃湿热型痤疮效果显著,随访复发率较低,值得进一步临床推广。

Objective： To observe the clinical effect of external application of Dan-Zhi herbal Compounds treating acne vulgaris in adolescents diagnosed as syndrome of damp-heat stagnation in lung and stomach. Methods： We enrolled 134 patients who were suffering from 1 -3 level heat-dampness accumulation in lung and stomach pattern of acne vulgaris. They were randomly divided into trial group and control group. 68 cases for trial group were given the Dan-Zhi herbal compounds for external application at affected part. 66 cases for control group were given the tretinoin cream, erythromycin and zinc Acetate Gel for external application at affected part. A course of treatment costs three weeks. At the end of the second course, we evaluate the clinical effect, four weeks later we did the fol- low-up visit. Results： There were significant differences between the two groups in variance of skin lesions at affected part at the end of the second course （P 〈0.05）, the total effective rate of trial group is 92. 65% （64/68） which is higher than 71.21% （46/66） of control group. There was no significant difference between the two groups in recurrence rate according to our follow-up visit （ P 〉 0. 05 ）. The recurrence rate of trial group is 15.63% （10/64） while control group is 36. 96% （17/46） . Conclusion： external application of Dan-Zhi herbal compounds exerts a very obvious effect on acne vulgaris in adolescents diagnosed as syndrome of damp-heat stagnation in lung and stomach. This treatment exhibits advantage at simple operation, low recurrence rate. It is worth of further in- depth research and can be widely spreaded in clinical application.
