经腹腔镜与开腹子宫肌瘤剔除术对比观察

Treatment Effect Comparison of Laparoscopic Myomectomy and Transabdominal Myomectomy to Hysteromyoma

目的了解经腹腔镜子宫肌瘤剔除术(laparoscopic myomectomy,LM)与开腹子宫肌瘤剔除术(transabdominal myomectomy,TAM)治疗子宫肌瘤的临床效果。方法选取100例子宫肌瘤患者,随机数字法分为观察组和对照组,各50例,对照组给予开腹子宫肌瘤剔除术治疗,观察组给予腹腔镜子宫肌瘤剔除术治疗,比较两组患者的临床治疗效果和术后妊娠情况。结果观察组住院时间(4.70±1.13)d短于对照组(7.90±1.50)d,差异有统计学意义(P<0.05);两组患者手术时间和术中出血量比较差异无统计学意义(P>0.05);术后观察组并发症发生率8.0%低于对照组22.0%,差异有统计学意义(P<0.05);术后观察组肌瘤残留率4.0%低于对照组14.0%,差异有统计学意义(P<0.05);两组患者术后复发率和妊娠率比较,差异无统计学意义(P>0.05)。结论 LM和TAM均可有效治疗子宫肌瘤,但LM在术后并发症发生率、肌瘤残留率等方面优于TAM,疗效安全可靠。

Objective To investigate the clinical effect of laparoscopic myomectomy and transabdominal myomectomy to hysteromyoma. Methods Collecting 100 cases of patients with uterine fibroids,and randomly divided into observation group（ LM） and control group（ TAM）. The 50 cases in control group were given abdominal uterine fibroids rejecting operation treatment,observation group was given laparoscopic uterine fibroids rejecting operation treatment. The writer compared two groups of patients clinical treatment effect and postoperative pregnancies. Results The LM group average hospitalization time was shorter（ 4. 70 ± 1. 13） d than TAM（ 7. 90 ± 1. 50） d,There was statistically significant difference（ P 〈0. 05）. Two groups of patients with average operation time and bleeding amount of average comparative differences had no statistical significance（ P 〉 0. 05）. LM postoperative complication rate was lower than the TAM group. The difference was statistically significant（ P 〈 0. 05）. LM group of myoma postoperative residual rate was lower 8. 0% than TAM group 22. 0%,difference had statistical significance（ P 〈0. 05）. The myoma residual rate of the observation group was lower（ 4. 0%） than the control group14. 0%. Differences between two groups of patients with postoperative recurrence rate and pregnancy rate had no statistical significance（ P 〉 0. 05）. Conclusion Laparoscopic uterine fibroids rejecting and abdominal uterine fibroids eliminate all can be effective treatment of uterine fibroids,but laparoscopic uterine fibroids operation in aspects such as the incidence of postoperative complications,fibroids retention rate is better than that of abdominal uterine fibroids out,safe and reliable curative effect,worth clinical promotion.
