经阴道子宫切口瘢痕妊娠病灶清除术的临床价值

The Clinical Value of Cicatricial Debridement by Vaginal Hysterectomy

目的探讨经阴道子宫切口瘢痕妊娠病灶清除术的临床价值。方法收集我院子宫切口瘢痕妊娠的妇女,随机分为研究组(接受阴式子宫切口瘢痕妊娠病灶清除术+子宫肌壁修补术)和对照组(接受腹腔镜手术)。对比两组手术指标、预后、住院时间、住院费用。结果两组妊娠囊大小、术前β-HCG及本次妊娠距上次时间比较差异无统计学意义(P>0.05);两组手术指标及术后实验室检查比较差异有统计学意义(P<0.05);研究组和对照组住院时间及住院费用分别比较差异有统计学意义(P<0.05)。结论经阴道子宫切口瘢痕妊娠病灶清除术疗效肯定,创伤小,可彻底清除病灶。

Objective To explore the clinical value of cicatricialdebridement by vaginal hysterectomy. Methods The women with scarpregnancy in the uterine incision were collected and randomly divided intothe study group （vaginal hysterectomy, scarring pregnancy clearance anduterine muscle wall repair） and the control group （received laparoscopicsurgery）. The operative index, prognosis, length of stay and hospitalizationexpenses were compared between the two groups. Results There was nosignificant difference in the size of the gestational sac, the preoperative-HCG and the interval between the two groups （P 〉 0.05）. There was astatistically significant difference between the two groups in operative indexand postoperative laboratory examination （P 〈 0.05）. The hospitalizationtime and hospitalization expenses of the study group and the controlgroup were statistically different （P 〈 0.05）. Conclusion The treatment ofcicatricial pregnancy by transvaginal uterine incision is effective, and thetrauma is small, and the focus can be removed completely.
