摘要
社区卫生机构在医疗卫生服务体系中承担着提供公共卫生服务和常见病、多发病基本医疗服务的重要功能,而社区医生队伍状况直接决定着这些功能的实现。本文从经济学视角出发,分析了社区医生具有的人力资本特性和社区卫生服务具有的正外部性特征,提出这些特性的存在导致社区医生先天供给不足,需政府对社区医生供给予以政策调节和政策干预。本文对上海社区医生队伍的数量和人力质量供给情况进行实证分析,发现政策干预效果不够理想,并分析指出当前政策干预对社区医生激励作用不足的原因,进一步提出增加社区医生收入、引入竞争机制、完善职业发展路径等对策建议。
With the health system reform,community health services play an important role in the medical services system,and the status of community doctor team has become a key condition. This paper discusses the economic characteristics of community doctor services,and points out that,due to the characteristics of human capital and the positive externalities of community health services, it will lead to the lack of community doctors and needs policy intervention. By the empirical analysis on the quantity and quality of Shanghai community doctor team these years,?further analysis points out that it is due to the policy not following the characteristics of human capital and lack of incentives for community doctor. Based on the above analysis,suggestions are proposed to increase the income of community doctors,introduce competition mechanism and improve career development system.
