上海地区糖尿病护理门诊开展现状调查与分析

Status of nurse-led diabetes clinics in Shanghai

目的调查分析上海地区糖尿病护理门诊开展现状。方法 2016年2—12月,调查上海地区开展糖尿病护理门诊的二级以上综合医院28所,由出诊护士完成调查问卷,内容涉及门诊基本信息、就诊患者信息、出诊护士信息和护士自身评价。结果门诊基本信息:28所医院的糖尿病护理门诊开设时间为0.5~9.0年,平均5.81年,中位开设时间5年;三级医院年门诊量12~480例次,中位数年门诊量120例次,二级医院24~1 200例次,中位数年门诊量150例次;患者教育/咨询时间10~120 min/例,中位咨询时间30 min/例。就诊患者信息:就诊患者中医师转介占64%,就诊病种并不局限于糖尿病,护理门诊所提供服务包括个体咨询、健康教育、行为指导和直接护理。出诊护士信息:出诊护士均为大专以上学历,高级职称占61%,由护士长兼护理门诊工作占71%,所有护士均通过糖尿病专科护理相关培训并获得合格证书。护士自身评价:所有护士对门诊工作满意,认为实现了自身价值并对今后发展充满期望。结论上海地区糖尿病护理门诊为患者提供了集咨询、教育、护理、管理为一体的综合服务,满足了患者多方面的健康需要,对临床治疗和疾病改善发挥了积极作用。

Objective To investigate the status of nurse-led diabetes clinics in Shanghai. Methods A total of28 hospitals in Shanghai were chosen from February to December 2016. The specialist nurses working in diabetes clinics were surveyed using a self-developed questionnaire. Results Diabetes clinics had been set up for 0. 5 to 9. 0 years,with an average of 5 years. The annual outpatient service quantity of the tertiary hospitals was 12-480,with an average of 120 years,while that of the secondary hospitals was 24-1 200,with an average of150 years. The consultation time was up to 10-120 min per person,with an average of 30 min per person. Most patients（ 64%） were referred by doctors. Specialist nurses provided extensive services such as individual counseling,health education,behavior guidance and direct nursing care about diabetes and other diseases.Specialist nurses working in diabetes clinics had junior college degree or above and qualification certificate of diabetes-related training. 61% of the participants had senior professional title and 71% also worked as head nurses. All specialist nurses were satisfied with the working status in diabetes clinics and showed great confidence in the future development. Conclusion The nurse-led diabetes clinics in Shanghai provide integrated services including consultation,education,nursing and management to meet the various health needs of patients and improve clinic outcomes.
