肺癌患者对化疗致周围神经炎症状心理体验的质性研究

Qualitative study on psychological experiences of lung cancer patients with chemotherapy-induced peripheral neuropathy

目的了解肺癌患者对化疗致周围神经炎(chemotherapy-induced peripheral neuropathy,CIPN)症状的心理体验,为制定针对性护理干预措施提供依据。方法采用便利抽样法,选取2015年11月—2016年2月,于同济大学附属上海市肺科医院住院化疗并伴有CIPN症状的肺癌患者12例,运用质性研究中的现象学研究法,进行半结构式深度访谈。采用Colaizzi 7步分析法进行资料整理分析,提炼主题。结果12例患者均完成访谈,对CIPN症状的心理体验共提炼出6个主题,包括认知不足、抑郁和悲哀等负性情绪、应对方法缺乏、生活能力下降、社会隔离以及希望得到更多专业指导和帮助。结论医护人员应关注化疗患者不良反应的主诉,指导患者对CIPN症状进行早期识别和处理,为患者提供针对性的治疗护理,帮助患者采取积极有效的应对方式,提高生活质量。

Objective To investigate the real psychological experiences of lung cancer patients with chemotherapy-induced peripheral neuropathy（ CIPN）, and provide a scientific basis for clinical nursing measures. Methods From November 2015 to February 2016,12 lung patients with CIPN were selected from Shanghai Pulmonary Hospital Affiliated to Tongji University. The phenomenological research method was used to perform in-depth semi-structured interviews. The themes of psychological experiences were extracted from the interview data,using Colaizzi data analysis principle. Results All cases completed the interviews,and 6 themes were extracted as follows： inadequate cognition,negative emotions including depression and sadness,lack of coping methods,declining in living ability,social isolation,expecting more professional guidance and help. Conclusion Medical staff should carefully pay attention to adverse reactions from patients with chemotherapy,instruct them to recognize early and actively cope with CIPN,and provide effective treatment to help relieve numbness and pain in order to improve their quality of life.
