全胃切除Roux-en-Y方式重建消化道治疗胃底贲门癌的临床研究

Clinical Study of Total Gastrectomy with Roux-en-Y Reconstruction of Digestive Tract in the Treatment of Gastric Fundus and Cardia Carcinoma

目的:探讨全胃切除Roux-en-Y方式重建消化道治疗胃底贲门癌的临床研究情况。方法:选取某院2012年1月~2016年6月符合实验条件的胃底贲门癌进行全胃切除Roux-en-Y方式重建消化道患者30例作为观察组,将以往进行近端胃切除的胃底贲门癌患者30例作为对照组,比较两组患者的临床疗效情况。结果:观察组术后1个月、6个月、12个月、18个月、24个月生活质量评估均显著高于对照组;两组在手术时间方面无显著性差异,观察组住院总费用、住院时间较对照组均有显著性优势;两组在术后并发生症发生率方面无显著性差异,P>0.05;观察组术后1年、2年存活率均显著高于对照组,P<0.05。结论:全胃切除Roux-enY方式重建消化道治疗胃底贲门癌具有术后生活质量高、存活率高的显著优势,并在手术操作上具有利于临床普及,不增加患者经济负担的特点。

Objective：To probe into the clinical research of total gastrectomy and Roux-en-Y way of reconstruction of digestive tract in the treatment of gastric fundus and cardia cancer.Methods：30 cases of patients with digestive tract and who conformed to the gastric fundus cardia cancer and underwent total gastrectomy and Roux-en-Y reconstruction in a hospital from January 2012 to June 2016 were selected as the observation group.30 cases of patients with gastric fundus and cardia cancer and treated with proximal gastrectomy in a hospital were selected as the control group.Then the clinical effects of two groups of patients were compared.Results：The quality of life after the operation in 1 months,6 months,12 months,18 months and 24 months in the observation group was significantly higher than that in the control group.There was no significant difference in the operation time between the two groups.The total cost of hospitalization and the hospitalization time in the observation group were significantly better than those in the control group.There was no significant difference in the incidence of symptoms after the operation between the two groups（P〈0.05）.The survival rate after operation in 1 and 2 years in the observation group was significantly higher than that in the control group（P〈0.05）.Conclusion：The total gastrectomy with Roux-en-Y reconstruction of digestive tract in the treatment of gastric fundus and cardia cancer has significant advantages of high quality of life and high survival rate after operation,which is beneficial to the clinical popularization of and does not increase the economic burden of the patients.
