振幅整合脑电图对羊水混浊患儿缺氧缺血性脑病的诊断价值

Diagnostic Value of Amplitude Integrated Electroencephalogram for Hypoxic Ischemic Encephalopathy in Children with Amniotic Fluid Turbidity

目的研究振幅整合脑电图(aEEG)对羊水混浊患儿缺氧缺血性脑病(HIE)的临床诊断价值。方法选取我院2016年8月至2017年7月收治的319例羊水混浊新生儿作为研究组,患儿分为轻度HIE和中度HIE。另纳入40例同期于我院进行健康体检的新生儿作为对照组。所有新生儿均进行aEEG检查,观察HIE临床分度与aEEG检查结果的关系,分析aEEG对HIE患儿的诊断价值。结果 319例患儿中,轻度HIE患儿304例(95.30%),中度HIE患儿15例(4.70%)。aEEG监测结果显示轻度异常241例,临界值63例,未见异常15例,aEEG异常率为75.55%。对照组的aEEG监测结果均显示未见异常,波幅分布范围,上线:25～50μV;下线:10～25μV。aEEG监测异常情况与HIE临床分度呈明显正相关(P<0.05)。结论 aEEG检测方法便捷、迅速,无创伤,可作为临床上早期诊断HIE的一种简便方法,以提高临床对HIE的早期诊断率。

Objective To study the clinical diagnostic value of amplitude integrated electroencephalogram（aEEG） for hypoxic ischemic encephalopathy（HIE） in children with amniotic fluid turbidity. Methods 319 cases of neonates with amniotic fluid turbidity admitted to our hospital from August 2016 to July 2017 were selected as study group and were divided into mild HIE and moderate HIE.Another 40 cases of neonates undergoing health examination in our hospital at the same time were selected as control group. All neonates received aEEG. The relationship between HIE clinical grading and aEEG examination results was observed, and the diagnostic value of aEEG in children with HIE was analyzed. Results Among 319 cases of children, 304 cases（95.30%） had mild HIE, and 15 cases（4.70%）had moderate HIE. aEEG monitoring results showed that there were 241 cases of mild abnormality, 63 cases of threshold and 15 cases of normal, with aEEG abnormal rate of 75.55%. aEEG monitoring results of the control group showed no abnormality, and amplitude distribution range, upline： 25～50 μV; downline： 10～25 μV. aEEG monitoring abnormality was positively correlated with HIE clinical grading（P 〈0.05）. Conclusions aEEG detection is convenient, rapid and untraumatic, which can be used as a simple method for early diagnosis of HIE, in order to improve the early diagnosis rate of HIE.
