分组管理护理干预对维持性血液透析患者血管通路的维护作用

Maintenance Effect of Grouping Management Nursing Intervention in Vascular Access of Patients with Maintenance Hemodialysis

目的探讨分组管理护理干预对维持性血液透析患者血管通路的维护效果。方法选取我院2016年8月至2017年9月收治的维持性血液透析患者98例,随机平均分为干预组(分组管理护理干预)和常规组(常规护理)。比较两组患者的URR、Kt/V、β_2-MG水平、并发症发生率、护理满意度。结果护理前,两组患者的URR、Kt/V、β_2-MG水平比较差异均无统计学意义(P>0.05);护理后,干预组的URR、Kt/V均高于护理前及常规组,β_2-MG水平低于护理前及常规组,差异均有统计学意义(P<0.05)。干预组的血管通路感染、狭窄、血栓、功能不良发生率均低于常规组(P<0.05)。干预组的护理满意度高于常规组(P<0.05)。结论分组管理护理干预对维持性血液透析患者血管通路具有显著的维护效果,可提高患者的透析质量,减少血管通路相关并发症,提高患者的护理满意度。

Objective To explore the maintenance effect of grouping management nursing intervention in vascular access of patients with maintenance hemodialysis. Methods 98 cases of patients with maintenance hemodialysis admitted to our hospital from August 2016 to September 2017 were selected and randomly divided into intervention group（grouping management nursing intervention） and routine group（routine nursing） equally. The URR, Kt/V and β2-MG level, incidence of complications and nursing satisfaction degree were compared between the two groups. Results Before nursing, no statistical difference was found in the URR, Kt/V and β2-MG level between the two groups（P 〉0.05）. After nursing, the URR and Kt/V of the intervention group were higher than those before nursing and those of the routine group, and the β2-MG level was lower than that before nursing and that of the routine group（all P 〈0.05）. The incidences of vascular access infection, stenosis, thrombosis and dysfunction of the intervention group were lower than those of the routine group（P 〈0.05）. The nursing satisfaction degree of the intervention group was higher than that of the routine group（P 〈0.05）. Conclusions Grouping management nursing intervention has significant maintenance effect in vascular access of patients with maintenance hemodialysis, which can improve the dialysis quality, reduce the vascular access-related complications, and improve patients’ nursing satisfaction degree.
