眼睑近泪小点良性肿瘤激光微创治疗

Laser minimally invasive treatment for benign eyelid tumor close to the lacrimal point

目的观察超脉冲CO_2激光治疗眼睑近泪小点良性肿瘤的效果。方法前瞻性临床研究。对2016年1月~2017年2月在本院眼耳鼻整形外科门诊就诊的24例眼睑近泪小点良性肿瘤患者行超脉冲CO_2激光微创切除术。所有患者完成调查问卷,包括:年龄、性别、病程、病变部位、病变形态以及治疗后并发症等。眼部检查包括眼前段照相、裂隙灯检查和泪道冲洗。随访时间为术后1周、1个月、3个月及半年。治疗时激光光斑大小为120~300μm,超脉冲CO_2激光能量密度为0.6~21.1 W/mm2。所有患者进行术后病理检查。结果术后创面干燥,无感染,2~4周后完全上皮化为正常皮肤。创面几乎无瘢痕及凹陷。所有患者泪小点保留完好,泪道通畅,无损伤。肿瘤均一次性去除,无一例并发症。结论超脉冲CO_2激光治疗眼睑近泪小点良性肿瘤安全、有效,可以在简便、微创去除肿瘤的同时完整保留泪小点,不损伤泪道,而且达到很好的美容效果,可以作为传统外科手术的一种替代方法。

Objective To evaluate the treatment of benign eyelid lesions close to the lacrimal point with a superpulse CO2 laser. Methods Prospective clinical study was used. Twenty-four patients who suffered from benign eyelidtumor close to the lacrimal point were treated with super pulse CO2 laser photocoagulation between January 2016 and February 2017. Before treatment, all patients received the usual clinical care, consisting of a case history a d ophthalmological examinations induing visual acuity,intraocular pressure,and a slit-lamp examination of the ey e l id s,anterior ocular segment,and syphilis of lacrimal passages. The a g e,s e x,follow-up time,operated eye and e y e l id,andthe number of lesions were recorded,including any postoperative complications or recurreweek,1 month,3 months and 6 months. Histological diagnoses were obtained in all patients. ResiUts The cosmeticoutcomes of ai patients were satisfactory ater treatment,and the wounds formed epithelialized within 2 to 4 weeks with normal-appearing epithelium. The treated area showA1 patients had intact tear spots and unobstructed lacrimal passages. No complications during the follow-up. Conclusions The super pulse CO2 laser therapy of the benign eyelid tumors close to the lacrimalpoint provided satisfactory cosmetic results in this study. It is a convenient,cheap,accessible, and well-tolerated alternative to the traditional surgery,especially for those positioned close to the lacrimal papillae.
