摘要
目的降低管路滑脱发生率,提升管道护理质量。方法完善预防管路滑脱闭环管理工作流程,包括动态评估、规范处理、实时上报、持续追踪改善和标准化维持;利用信息化平台实现关键指标监测。结果管路滑脱每千床日数发生率从2015年的0.0189‰下降至2016年的0.0131‰;2016年预防管路滑脱危险因素评估准确、管路固定正确、健康教育落实、身体约束安全、交接班5项关键指标落实率逐步提升。结论闭环管理联合关键指标监测,可有效降低管路滑脱的发生,提升管理效能。
Objective To reduce the incidence of unplanned extubation and to improve quality of catheter care.Methods Closed-loop workflow for prevention of unplanned extubation was improved,including such procedures as dynamic assessment,standardized management,real-time reporting,continuous tracking and reaction,as well as normalized maintenance.And key indicators were monitored by using information technology.Results The incidence of unplanned extubation was decreased from 0.0189‰in 2015 to0.0131‰in 2016.Five key indicators as correct risk factor assessment,correct catheter securement,execution of health education,safe physical restraint use,and handover were gradually improved in 2016.Conclusion Combination use of closed-loop management and key indicator monitoring can reduce unplanned extubation and improve management efficiency.
