早期气管切开对老年重型脑室出血患者的疗效观察

Clinical observation of early tracheotomy on elderly patients with severe ventricular hemorrhage

目的观察早期气管切开对老年重型脑室出血患者的疗效影响。方法选取我院60例老年重型脑室出血患者,随机分为观察组和对照组(各30例),观察组患者入院后急诊行脑室穿刺外引流术时即同时行经皮扩张切开术,对照组患者出现血氧饱和度低时再行经皮扩张气管切开术。观察肺部感染发生率、住院时间及发病3个月的治疗效果等。结果观察组患者治疗有效率和对照组比较有统计学差异(P<0.05),肺部感染发生率及住院时间比较均有统计学差异(P<0.05)。结论对于老年重型脑室出血患者,尽早行气管切开术可以改善治疗效果。

Objective The effect of early tracheotomy on elderly patients with severe ventricular hemorrhage was investigated.Methods A total of 60 elderly patients with severe ventricular hemorrhage in our hospital were selected.They were randomly divided into the observation group and the control group with 30 in each group.The patients in the observation group underwent percutaneous dilational tracheostomy and bilateral ventriculostomy at the same time after emergency admission.In the control group,percutaneous dilatational tracheostomy was pefformed when the blood oxygen saturation was low.Incidence of pulmonary infection,admission time,and prognosis after 3 months in each group were observed.Results The effective rate,the incidence of pulmonary infection and admission time in the observation group was significantly different from that of the control group.Conclusion Tracheotomy as early as possible can improve the treatment effect for the elderly patients with severe ventricular hemorrhage.
