系统性红斑狼疮血尿酸与肾损害相关因素的临床研究 被引量:1

Clinical study on the relationship between serum uric acid level and renal damage in patients suffered systemic lupus erythematosus

摘要 目的探讨血尿酸(SUA)水平与系统性红斑狼疮(SLE)患者肾损害的关系及对肾损害的诊断价值。方法选择于齐齐哈尔市第一医院住院或门诊SLE患者230例及健康体检人群100名。检测患者血清补体C3、血尿酸、血肌酐(Scr),评估疾病活动性指数(SLEDAI评分)。结果 SLE组血尿酸水平及肾脏损害明显高于正常对照人群;狼疮性肾炎(LN)组血尿酸水平、补体、SLEDAI评分、抗ds-DNA抗体与非狼疮性肾炎(NLN)组比较有统计学差异;SLE中高尿酸血症(HUA)组与非高尿酸血症(NHUA)组比较血肌酐、尿素氮(BUN)、补体、抗ds-DNA抗体、SLEDAI评分有统计学差异;血尿酸与病程、年龄无相关性,与血肌酐、尿素氮、尿蛋白、尿潜血、补体C3、抗ds-DNA抗体、SLEDAI评分及血压相关。结论血尿酸水平与系统性红斑狼疮肾损害有相关性,血尿酸水平升高是肾损害的危险因素。 Objective To investigate the relationship between the level of serum uric acid and renal damage in patients with SLE and the diagnostic value for rental damage in patients with SLE.Methods A total of230 cases of hospitalized or outpatient SLE patients and 100 cases of health examination population in the first hospital of Qiqihar were chose as study subjects. The concentration of serum alexin C3,serum uric acid,and serum creatinine( Scr) was detected,and assessed the disease activity index( SLEDAI score). Results Serum uric acid level and renal damage in SLE group were significantly higher than those in normal control group. There were significant differences in serum uric acid level,complement,SLEDAI score and anti ds-DNA antibody between lupus nephritis group and non-lupus nephritis group. The serum creatinine, urea nitrogen bun,complement and anti ds-DNA antibody scores were significantly different between the hyperuricemia group and the non hyperuricemia group. There was no correlation between serum uric acid and course of disease,age. The level of SUA was correlated with blood creatinine,urea nitrogen,urine protein,urine occult blood,complement,anti-ds-DNA antibody,SLEDAI score and blood pressure.Conclusions Serum uric acid level was associated with renal damage in SLE,and the elevated uric acid level was a risk factor for renal damage.
