腔镜经胸乳入路与低位小切口甲状腺手术的临床疗效比较

The different clinical effects of endoscopic thyroid surgery via a breast approach and lower mini-incision thyroidectomy

目的:探讨经胸乳入路腔镜甲状腺手术与低位小切口手术的临床疗效。方法:选取2015年8月至2017年8月诊断为良性甲状腺结节的260例患者,其中130例经胸乳入路行腔镜甲状腺手术(腔镜组),130例行低位小切口甲状腺手术(小切口组)。比较两组患者术前临床资料、术后并发症及美容效果。结果:两组患者年龄、性别、肿块大小、住院时间及并发症发生率差异无统计学意义;腔镜组出血量、引流量及引流管放置时间少于小切口组,但手术时间长于小切口组,差异有统计学意义(P<0.05);腔镜组美容效果更佳。结论:经胸乳入路行腔镜甲状腺手术与低位小切口甲状腺手术均具有较好的临床疗效,但腔镜手术的美容效果更佳。

Objective：To investigate the different clinical effects of endoscopic thyroid surgery via a breast approach and lower mini-incision thyroidectomy. Methods：A total of 260 cases of benign thyroid nodules were selected from Aug. 2015 to Aug. 2017 and were divided into the endoscopic thyroid surgery group（ n = 130） and the mini-incision thyroidectomy group（ n = 130）. Clinical characteristics,postoperative complications and cosmetic effect were compared between the two groups. Results：There were no differences in age,gender,nodule size,postoperative hospital stay and postoperative complications between the two groups. The blood loss,amount of drainage and duration of drainage of the endoscopic thyroid surgery group were obvious less than those of the mini-incision thyroidectomy group,but the operation time of the endoscopic thyroid surgery group was obvious longer than that of the mini-incision thyroidectomy group（ P〈 0. 05）. The cosmetic effect of the endoscopic surgery group was better than that of the mini-incision thyroidectomy group.Conclusions：Both endoscopic thyroid surgery via a breast approach and lower mini-incision thyroidectomy are practicable for patients,but the cosmetic effect of endoscopic thyroid surgery turns out to be better.
