摘要 目的:探究Fazekas量表评分评估不同程度脑白质损害与脑电图改变的相关性。方法:本研究纳入门诊诊断为"晕厥"的患者,采用Fazekas量表评分评估头颅MRI脑白质高信号严重程度,患者同时进行16导联脑电监测,分析脑电慢波表现与不同程度脑白质高信号相关性。结果:本研究共入组641例患者,Fazekas量表评分5~6分的患者在18~44岁、45~64岁、65~79岁、80~92岁年龄组中所占的比重分别为0.0%、3.9%、11.7%、28.8%,各组间差异均为P<0.05。Fazekas量表评分1~6分组患者中有116例(35.3%)出现脑电异常,与Fazekas 0分组的79例(25.3%)相比较,差异有统计学意义(P=0.006)。Fazekas 5~6分组内出现脑电慢波发放增多的比例为56.5%,同1~2分组(29.5%)相比,差异具有统计学意义(P<0.05)。结论:脑白质高信号严重程度与脑电慢波发放显著相关,Fazekas量表评分为5~6分患者的脑电监测结果需分析其与疾病症状的相关性,以早期发现脑功能改变情况,并给予相应的干预处理。 Purpose: To study the correlation of the severity of white matter hyperintensities (WMHs) and the changes of electroencephalogram. Methods: The patients diagnosed as "syncope" in neurology clinic were included in our study. The severity of the WMHs on MRI was assessed by the visual rating of Fazekas scale. The patients were also performed the 16-lead EEG monitoring. The correlation between electroencephalogram and the severity of WMHs was analyzed. Results: A total of 641 patients were included in our study, the proportion of patients with Fazekas 5-6 in the18-44, 45- 64, 65-79, 80-92 years old group was 0.0%, 3.9%, 11.7%, and 28.8%, respectively, and the differences between each two groups were with statistically significant, P〈0.05. One hundred and sixteen patients (35.3%) of the Fazekas 1-6 group, and 79 patients (25.3%) of the Fazekas 0 group (P=0.006) were detected with abnormal EEG. The proportion of the increased slow wave discharge in the Fazekas 5-6 group was 56.5%, which was significantly higher than that (29.5%) in the Fazekas 1-2 group (P〈0.05). Conclusion: The severity of the WMHs is significantly associated with the slow wave discharge. Attention should be paid to analyzing the correlation of patients' electroencephalogram and the clinical symptoms to early detect the neuro-electrophysiological changes and take early intervention.
