邢台市基层医护人员乙型肝炎病毒表面抗体水平及其影响因素

The level of HBsAb and related influencial factors in medical and nursing staff members in Xingtai City

目的了解邢台市基层医护人员乙型病毒性肝炎(简称乙肝)病毒表面抗体(hepatitis B surface antibody,HBs Ab)水平及其影响因素。方法 2017年8月,随机抽取邢台市19个县市区38家乡级卫生院760名健康基层医护人员作为调查对象,采集每名调查对象清晨空腹静脉血4~5 m L,分离血清,采用ELISA检测每名调查对象空腹血清中HBs Ab浓度,用SPSS 20.0软件对相关数据进行统计学分析。结果 760名基层医护人员HBs Ab阳性者391例,总阳性率为51.45%(弱阳性率为38.42%,阳性和强阳性率为13.03%);≤25岁年龄组的基层医护人员HBs Ab水平明显高于25岁以上年龄组(χ~2=40.081,P<0.001);有免疫史组的基层医护人员HBs Ab水平明显高于无免疫史组(χ~2=103.306,P<0.001);不同性别、不同学历、不同地区的基层医护人员HBs Ab水平差异均无统计学意义(χ~2=3.443、χ~2=4.133,χ~2=22.222,P>0.05)。结论目前邢台市基层医护人员的HBs Ab水平不高,建议对每年新入职的人员开展HBs Ab检测,对抗体阴性者及时进行乙肝疫苗补种,从而提高基层医护人员HBs Ab水平。

Objective To investigate the status and influencial factors of hepatitis B surface antibody（ HBs Ab） level of hepatitis B virus（ HBV） in primary health care staff members in Xingtai City. Methods In August 2017,38 clinnics were randomly selected from 19 Townships in Xingtai City,760 health primary medical staff members as the objects,each collected blood 4-5 m L and serum separated,HBs Ab level was detected by ELISA and the data analyzed by using SPSS 20.0 software. Results There were 391 HBs Ab positive among 760 tested staff members,accounted to 51.45%（ weak positive38.42%,both of positive and strong positive 13.03%）; the data of HBs Ab levels in ≤25 years age group was significantly higher than that of 25 age group（ χ^2= 40.081,P 〈0.001）; the HBs Ab level of the vaccinated group was statistically significant higher than that of non-vaccinated group（ χ^2= 103.306,P 0.001）; the differences in the HBs Ab levels had no a statistical significance（ χ^2= 3.443,χ^2= 4.133,χ^2= 22.222,P 〈0.05） in different genders and different degrees. Conclusion The HBs Abs in some primary medical and health care staff members in Xingtai City was not in a standard level,it suggested that the detection of HBs Ab should be carry out in new members each year,and maintain hepatitis B vaccination for antibody negative ones in due time,so as to improve the level of HBs Ab in the standard level.
