摘要
目的了解四川大学华西医院就诊患者弓形虫(Toxoplasma gondii,TOX)感染情况,为制定TOX病预防措施提供科学依据。方法选取2015-01/2017-02到四川大学华西医院进行TOX抗体(Ig G,Ig M)筛查的门诊及住院就诊者共15 412例,其中抗体阳性患者1 159例,通过HIS及LIS获得抗体阳性患者相关病例及信息。结果 TOX抗体阳性率为7.52%,76.27%为正常免疫人群感染,免疫缺陷和风湿免疫患者抗体阳性率亦较高;41~60岁人群弓形虫阳性率最高(9.62%),而20岁以下人群阳性率率最低(4.94%),且差异有统计学意义(P=0.003),抗体阳性者男性共占54.70%,大于女性(45.30%),但差异均无统计学意义(P>0.05)。农民、退休及无业人员弓形虫抗体阳性率较高、职业、主诉及免疫力正常与否间存在差异。结论四川大学华西医院TOX抗体阳性率与全国平均水平相近,年龄、职业、免疫缺陷是其感染重要因素,近80%为无症状隐性感染提示TOX抗体筛查对预防及减少TOX相关疾病均尤为重要。
Objective To investingate the positive rate of Toxoplasma gondii (TOX) antibody in patientsin West China Hospital of Sichuan University and analyze the influencing factors, providing the scientific evidence for effective prevention strategy of TOX. Methods The study selected 15 412 outpatients and inpatients who received the TOX antibody (IgG, IgM) screening from January 2015 to February 2017 as subjects, among them, 1 159 cases were antibody positive patients and their related cases and information were obtained by HIS and LIS. Results The positive rate of TOX antibody was 7.52%, 76. 27% of which were asymptomatic infection, patients with immunodeficiency or rheumatic autoimmune disease showed high positive rate of TO)( antibody. The positive rate of TOX antibody was the highest in 41 -60 years old patients (9.62%), while the lowest positive rate lied in patientsunder 20 years old, the difference between the two groups was statistically significant (P 〈 0.05 ). There were 54. 70% of the male with positive antibody, which was higher than the female (45.30%), but the difference was not statistically significant (P 〉0.05 ). Farmers, the retired, and the unempolyed had higher positive rate of TOX antibody. There were differences between patients with different occupations, patient complaints, and situation of immune system. Conclusion The positive rate of TOX antibody of patients in West China Hospital of Sichuan University was close to the national average level. Age, occupation, immunodeficiency were the important influencing factors for infection, the nearly 80% asymptomatic infection highlighted the importance and necessity of TOX screening to the prevention and reduction of TOX related diseases.
