硬式支气管镜治疗气管血管球瘤1例并文献复习

One case with glomus tumor of the trachea treated by rigid bronchoscopy and literature review

目的探讨气管血管球瘤（GT）的临床表现、病理特点和治疗方法。方法对遵义医学院附属深圳市龙岗中心医院收治的1例经CT和病理确诊的气管GT病例进行回顾性分析,并结合文献进行讨论分析。结果患者女性,63岁,表现为咳嗽、咳痰、气促,听诊可闻及吸气相哮鸣音,胸部CT见主气管后壁软组织影向气管内突出,通过硬式支气管镜切除肿物,病理结果确诊为GT。随访1年未见肿瘤复发。结论气管GT非常罕见,通常是良性病变,易被误诊为其他肺部病变,确诊主要依靠病理,需结合免疫组织化学,硬式支气管镜治疗安全,创伤小,无需切除气管,可作为治疗良性气管GT的新方法。

Objective To investigate the clinical manifestations, pathological features and treatment of glomus tumor of the trachea. Methods A case of glomus tumor of the trachea confirmed by pathological admitted to our hospital was analyzed and the related Literatures were reviewed. Results 63 years old female patient had cough, expectoration, shortness of breath, auscultation could heard inspiratory wheeze, chest CT showed prominent wall soft tissue shadow to the trachea, treatment with resection of the tumor through rigid bronchoscopy and pathological results confirmed GT, and follow-up did not reveal tumor recurrence 1 year after the operation. Conclusions Glomus tumor of the trachea are extremely rare and generally benign lesions, which is easy to be misdiagnosed to other pulmonary diseases, The final diagnosis depends on pathological findings, combined with immunohistochemistry, bronchoscopic treatment is more safety, little trauma, without resection of the trachea, can be used as a new method for the treatment of benign glomus tumor of the trachea, but because of few cases were treated by bronchoscopy, the new treatment need to accumulate more experience.
