膝关节镜治疗不同KL分级膝关节运动性损伤的疗效分析

Curative effect analysis of arthroscopic treatment for different KL graded knee joint sports injuries

目的：探讨膝关节镜在治疗不同Kellgren-Lawrence（KL）分级膝关节运动损伤性疾病中的效果及安全性,为临床疾病治疗提供依据.方法：选取2015年9月～2017年9月于我院就诊并行膝关节镜治疗膝关节运动性损伤患者108例为研究对象.按照KL分级标准及不同疾病种类将患者分类,所有患者均行膝关节镜进行治疗.观察患者手术前后膝关节Lysholm功能评分情况并进行统计学分析.结果：KL分级0级、Ⅰ级及Ⅱ级患者术后Lysholm功能评分显著高于手术前（P〈0.05）,而KL分级Ⅲ级患者手术前后Lysholm功能评分相比差异不明显（P〉0.05）.疾病种类为膝关节关节软骨及半月板磨损、半月板撕裂伤及膝关节交叉韧带断裂伤患者手术后Lysholm功能评分明显高于术前（P〈0.05）,膝关节滑膜病变患者手术前后Lysholm功能评分相比差异不明显（P〉0.05）.结论：不同KL分级及不同疾病种类膝关节运动损伤行膝关节镜手术疗效不同,临床应注意区分.

Objectiv： To explore the Curative effect analysis of arthroscopic treatment for different KL graded knee joint sports injuries and provide the basis for clinical treatment. Methods： A total of 108 cases of patients with knee joint sports injuriesfrom September 2015 to September 2017 in our hospital were studied. According to the KL grading rules and the disease types, they were divided into different groups. All the patients were given arthroscopic treatment , the Lysholm function scores were observed before and after the surgery and statistical analysis was performed. Results ： Lysholm function score was significantly higher in KL grade 0, grade Ⅰ and Ⅱ patients than before surgery （ P 〈 0.05 ）. However, there was no significant difference in Lysholm function score before and after the operation of KL grade Ⅲ patients （ P 〉 0.05 ）. Diseases of knee articular cartilage and meniscus laceration of wear and tear, meniscus and the broken knee cruciate ligament Lysholm function score was significantly higher after surgery in patients with preoperative （ P 〈 0.05 ） , Lysholm knee joint synovial disease patients with functional score difference was not obvious （ P 〉 0.05 ）. Conclusion： Different KL grades and different kinds of diseases might have different effects on knee arthroscopic surgery, clinical attention should be distinguished.
