晚期肝癌患者主要照顾者的负担、焦虑和抑郁现状及影响因素分析

Study on the Status of Burden,Anxiety and Depression and the Influencing Factors of Primary Family Caregivers of Patients with Advanced Hepatocellular Carcinoma

目的:探讨晚期肝癌患者主要照顾者的负担状况,并分析其主要的影响因素,以便制定针对性的干预措施,减轻照顾者的负担,改善其生活质量。方法:本研究选取住院治疗的晚期肝癌患者主要照顾者为调查对象。对符合入选标准的204例患者的照顾者进行问卷调查,使用照顾者反应评估量表(CRA)、照顾者焦虑抑郁量表(HADS)和自编的一般情况调查表分别评估照顾者的压力反应和生活质量。采用SPSS19.0软件对采集的数据进行统计分析。结果:晚期肝癌患者主要照顾者的负担得分按平均值由大到小排序依次为:经济负担(3.52)>时间安排受打扰(2.95)>健康状况(2.74)>缺乏家庭支持负荷(2.47)>自尊维度(2.09)。照顾者的家庭经济负担和时间安排受到打扰是主要的相关影响因素。结论:晚期肝癌患者主要照顾者普遍存在中度照顾负担,医护人员在护理患者的同时应将照顾者经济收入水平、身体健康状况等内容纳入护理评估范围,并采取相应措施以缓解照顾者的负担。

Objective To explore the burden of primary caregivers in patients with late term hepatocellular carcinoma,and to analyze the main influencing factors,so as to formulate targeted intervention measures to reduce the burden of caregivers and improve their quality of life. Methods In this study,the main caregivers of patients with advanced liver cancer who were hospitalized were select as the subjects. A questionnaire survey was conducted on 204 caregivers who met the inclusion criteria. Caregiver Response Rating Scale（ CRA）,Caregiver Anxiety and Depression Scale（ HADS） and self-compiled general questionnaires were used to assess the caregiver＇s response to stress and quality of life. Software SPSS19.0 was used to analyze the collected data. Results The burden of major caregivers in the patients with advanced hepatocellular carcinoma ranked in descending order of average as follows： economic burden（ 3.52）〉 disturbed by timing（ 2.95） 〉health status（ 2.74）〉 lack of family support load（ 2.47）〉 self-esteem dimension（ 2.09）. Disturbances in the caregivers＇ family financial burdens and the timing of their interventions are the main relevant influencing factors. Conclusion The main caregivers of patients with advanced hepatocellular carcinoma generally have a moderate care burden. While nursing caregivers,the level of economic income,physical health and other content of caregivers should be taken into the scope of care assessment,and take appropriate measures to ease the burden on the caregivers.
