丙氨酰谷氨酰胺对体外循环VR的肺保护作用

摘要 目的探讨丙氨酰谷氨酰胺对体外循环瓣膜置换术(VR)患者的肺保护作用。方法选择中山大学附属第八医院(深圳福田)收治的行体外循环VR患者66例进行研究,按照随机数字表法分成观察组和对照组各33例,对照组实施复方氨基酸治疗,观察组实施丙氨酰谷氨酰胺治疗,观察两组炎症反应、呼吸指数等变化。结果治疗后观察组与对照组炎症因子水平、肺泡气-动脉血氧分压差及呼吸指数均升高,其中观察组升高幅度小于对照组,差异有统计学意义(P<0.05)。结论在体外循环VR治疗中应用丙氨酰谷氨酰胺可进一步减轻患者的炎症反应,保护肺功能,促使患者尽快康复,值得推广应用。 Objective To investigate the lung protective effect of alanyl-glutamine on patients undergoing cardiopulmonary bypass valve replacement(VR). Methods 66 patients undergoing cardiopulmonary bypass VR in the Eighth Affiliated Hospital of Sun Yat-sen University(Shenzhen Futian) were selected and divided into control group and observation group, according to the random number table with 33 cases in each group. The patients in the control group were given compound amino acid therapy. The patients in the observation group were treated with alanyl-glutamine, the changes in inflammatory response, respiratory index and other changes were observed in two groups. Results There were increased in the level of inflammatory factors, alveolar-arterial oxygen difference and respiratory index in the two groups after treatment. The change range of the observation group was smaller than that of the control group, the differences were statistically significant(P〈0.05). Conclusion The application of alanyl-glutamine in the treatment of cardiopulmonary bypass VR can further reduce the inflammatory reaction, protect the lung function and promote the recovery of the patients as soon as possible, which deserves promotion.
