差异性切面的超声心动图对胎儿心脏畸形的诊断价值

Value of Differential Section Echocardiography in the Diagnosis of Fetal Heart Malformations

目的:探讨差异性切面的超声心动图对胎儿心脏畸形的诊断价值。方法:以眉山市妇幼保健院产科检查的900名中晚期妊娠妇女为对象,行差异性切面超声心动图检查,以引产后尸检及分娩后新生儿心脏超声诊断为金标准,评估差异性切面的超声心动图对胎儿心脏畸形的诊断价值。结果:共检出先天性心脏病16例、心律失常5例、心肌病2例、心功能不全1例、心脏肿瘤1例;与随访金标准相比较,有1例先天性心脏病被漏诊,差异性切面超声心动图诊断胎儿心脏畸形灵敏度、特异度为96.15%、100%;共检出10例复杂型心脏畸形,差异性切面超声心动图诊断复杂心脏畸形的灵敏度、特异度分别为85.27%、98.57%。结论:差异性切面超声心动图在胎儿心脏畸形诊断中有较高灵敏度和特异度,在鉴别心脏畸形的类型中有一定积极作用。

Objective： To investigate the value of differential section echocardiography in the diagnosis of fetal heart malformations. Methods： 900 women in the second or third trimester of pregnancy who received obstetric examination in our hospital were selected as subjects, and differential section echocardiography was performed. The value of differential section echocardiography in the diagnosis of fetal heart malformations was evaluated with autopsy after induction of labor and neonatal cardiac ultrasonic diagnosis after delivery as the golden standard. Results： A total of 16 cases of congenital heart disease, 5 cases of arrhythmia, 2 cases of cardiomyopathy, 1 case of heart insufficiency and 1 case of cardiac tumor were detected. Compared with the follow-up golden standard, there was 1 case of congenital heart disease never diagnosed. The sensitivity and specificity of differential section echocardiography in the diagnosis of fetal heart malformations were 96.15% and 100% respectively. A total of 10 cases with complex heart malformations were detected, and compared with the golden standard, the sensitivity and specificity of differential section echocardiography in the diagnosis of complex heart malformations were 85.27% and 98.57% respectively. Conclusion： The sensitivity and specificity of differential section echocardiography are high in the diagnosis of fetal heart malformations, and it plays a positive role in identifying the type of heart malformations.
