头颈部受区静脉逆向吻合在游离皮瓣中的应用 被引量:2

摘要 目的探讨显微皮瓣手术中单条静脉近、远端同时做为受区血管进行吻合的手术效果。方法2010年6月至2014年8月,共15例行游离皮瓣移植.其中头皮和颅骨缺损5例.颈部Ⅲ度以上瘢痕挛缩4例,乳腺癌手术后组织缺损6例。头皮和颅骨缺损采用旋髂浅动脉皮瓣和旋髂深动脉髂骨瓣的双蒂复合瓣修复.颈部瘢痕挛缩采用双蒂组合的肩胛皮瓣和背阔肌穿支皮瓣修复。乳腺癌乳房扩大切除术后用双蒂的DIEP修复。15例均采用两套静脉系统,即同1条静脉的远端和近端同时作为受区静脉进行吻合.以确保皮瓣血液回流通畅。其中受区利用颞浅静脉2例,颞中静脉3例,面静脉4例.胸廓内静脉6例。结果术中吻合静脉开放后血流通畅,皮瓣颜色良好。术后3个月随访,皮瓣均成活良好,皮瓣饱满而不臃肿,色泽、质地良好。未见静脉淤滞、血管异常扩张等情况。结论单条静脉近、远端同时应用虽然使一端受区静脉逆向吻合.但不影响皮瓣的成活,且缩短了手术时间.临床效果良好。
