胱抑素C、NGAL联合内皮素-1检测在高血压病早期肾损伤诊断中的价值研究

The Value of Joint Detection of Cystatin C,NGAL and Endothelium-1 in the Diagnosis of Hypertension with Early Renal Injury

目的研究三项指标联合检测在高血压病早期肾损伤诊断中的应用价值。方法选择2015年9月至2016年12月就诊于我院的高血压患者70例,根据内生肌酐清除率将所有患者分为单纯性高血压组32例,高血压肾损伤组38例。两组均接受胱抑素C、中性粒细胞明胶酶相关脂质运载蛋白(neutrophil gelatinaseassociatedlipocalin,NGAL)及内皮素-1检测。比较两组上述三项指标水平,并评估三项指标单一检测与联合检测高血压病早期肾损伤的临床价值。结果高血压肾损伤组胱抑素C、NGAL、内皮素-1水平均明显高于单纯性高血压组,差异有统计学意义(P<0.05);三项指标联合检测高血压早期肾损伤的灵敏度、特异性均明显高于单一检测,差异有统计学意义(P<0.05)。结论三项指标联合检测在高血压病早期肾损伤诊断中具有显著价值,有助于早期发现肾损伤,及时治疗,改善预后。

Objective To study the application value of joint detection of three indexes in the diagnosis of hypertension with early renal injury. Methods 70 patients with hypertension in our hospital from September 2015 to December 2016 were selected. And they were then divided into pure hypertension group （n = 32） and hypertension with renal injury group （n = 38 ） according with the creatinine clearancerate. Both groups were using joint detection of cystatin C, neutrophil gelatinaseassociated lipocalin （NGAL） and endothelium- 1. The levels of these three indexes and positive rate of two groups were then compared. The clinical values of single detection and joint detection of three indexes in diagnosis of hypertension with early renal injury were further evaluated. Results The cystatin C, NGAL, endothelium-1 levels in hypertension with renal injury group were significantly higher than those of pure hypertension group （ P 〈 0.05 ） ; The sensitivity and specificity of joint detection of three indexes were higher than those of single detection （ P 〈 0.05 ）. Conclusion Joint detection of three indexes in the diagnosis of hypertension with early renal injury has significant value. It can be used for early detection of renal injury, facilitate timely treat and improve prognosis.
