超声检查诊断妊娠早期胎儿中枢神经系统畸形的价值 被引量:3

摘要 目的探讨超声检查对妊娠早期胎儿中枢神经系统(CNS)畸形的应用价值。方法选取2013年6月至2016年12月妊娠早期孕妇2601例(2651胎),根据超声测量头臀长将其分为孕11~11^+6周组204例、孕12~12^+6周组1435例及孕13~13^+6周组962例。对所有妊娠早期孕妇行超声筛查,并对诊断为胎儿CNS畸形病例进行随访。观察分析三组胎儿CNS结构的显示情况及不同孕周对其影响,计算超声检查的敏感度。结果妊娠早期超声检查共检出胎儿CNS畸形13例,发生率0.49%;妊娠早期超声检查胎儿CNS的敏感度85.70%;孕11~11^+6周胎儿超声检查CNS显示率96.08%,孕12~12^+6周97.70%,孕13~13^+6周98.02%,三组比较差异无统计学意义(P〉0.05)。结论超声检查妊娠早期不同孕周的胎儿CNS结构显示率均较高,对诊断胎儿CNS畸形具有重要意义,值得在临床上推广使用。 Objective To explore the value of ultrasonography in detecting fetal central nervous system ( CNS ) deformity in early pregnancy. Methods A total of 2601 pregnant women ( 2651 fetuses ) admitted to our department from June 2013 to December 2016 were enrolled as the study object, according to the ultrasonic measurement of the first nnnp length, they were divided into pregnant 11~11^+6 weeks group ( n=204 ) , pregnant 12~12^+6 weeks group ( n=1435 ) and pregnant 13~13^+6 weeks group ( n=962 ) . All pregnant women in early pregnancy were given ultrasound screening, and the diagnosis of fetal CNS defornity cases were followed up. The display of fetal CNS structure in three groups and the influence of different gestafional weeks were observed and analyzed, the sensitiviW of tdtrasound examination was calculated. Results In this study, 13 cases of fetus CNS deformity were detected by ultrasonography in early pregnancy, the incidence was 4.90%; the sensifivity of fetus CNS during early pregnancy was 85.70%; pregnancy 11~11^+6 weeks fetal ultrasound exanaination CNS showed a rate of 96.08%, Pregnancy 12~12^+6 weeks 97.70%, pregnancy 13~13^+6 weeks was 98.02%, three groups of data was no significant difference ( P〉0.05 ) . Conclusion Ultrasound examination during the first trinaester gestafional age fetal CNS structure shows higher rates, the diagnosis of fetal is of great significance for CNS defonrtity, it is worth to promote the clinical use.
