2004-2016年四川省美沙酮维持治疗患者依从性及影响因素分析

The Compliance and Influencing Factors of Methadone Maintenance Therapy in Patients in Sichuan Province From 2004 to 2016

目的了解四川省美沙酮维持治疗情况及影响因素。方法回顾四川省美沙酮维持治疗数据库的数据和信息,以首次入组治疗的患者为研究对象,累计纳入25 685名患者,采用SPSS 22.0进行数据分析。结果截止2016-06-30,累计发生脱失15 931例,脱失比例62.0%。患者维持治疗平均时间30.2±30.5月,中位保持时间40.0月,患者6月、12月及18月累计保持率分别为0.80、0.72、0.66。多因素Cox回归分析结果显示,性别、文化程度、入组前近半年吸毒方式、维持治疗剂量、是否被公安抓捕过是患者维持治疗依从性的独立危险因素。结论四川省美沙酮维持治疗的保持情况还有待提高,加强吸毒者教育引导、提高美沙酮维持治疗剂量,对于促进其维持治疗具有重要意义。

Objective To understand the maintenance and treatment of methadone in Sichuan province and its influencing factors. Methods reviewed the Methadone Maintenance Treatment Database in Sichuan Province,Patients who were treated for the first time were included in the study（ 25 685 patients）. Data analysis was performed using SPSS 22. 0. Results As of June 30,2016,the cumulative loss was 15 931 cases; the proportion of the loss was 62. 0%. The mean duration of maintenance for the patients was 30. 2±30. 5 months,the median retention time was 40. 0 months,and the cumulative retention rate was 0. 80,0. 72 and 0. 66 in 6 th months,12 th month and 18 th month. Multivariate Cox regression analysis showed that gender,educational level,drug use methods in the recent half year before treatment,the maintenance of treatment dose and the public security arrest were the patient to maintain treatment compliance independent risk factors. Conclusion The maintenance of methadone in Sichuan province needs to be improved,it is important to strengthen the education and guidance of drug addicts and improve the dose of methadone maintenance therapy.
