摘要
目的分析血清外泌体miR-184在非小细胞肺癌中的表达水平,及其对非小细胞肺癌的诊断效能。方法选取非小细胞肺癌患者112例、肺炎患者60例、体检健康者40例作为研究对象,分别收集3组血浆标本,提取经纯化鉴定的血清外泌体中miRNA,采用QRT-PCR法检测3组血清外泌体miR-184的表达水平,利用ROC曲线评价血清外泌体miR-184在非小细胞肺癌中的诊断效能。结果本研究纯化的样品中含有大量外泌体颗粒且具备既往报道的外泌体特性;3组血清外泌体miR-184的表达量,组间两两比较差异均具有统计学意义(P<0.05),且非小细胞肺癌患者血清外泌体miR-184表达量明显高于肺炎患者和体检健康者(P<0.05);ROC曲线图下血清外泌体miR-184对非小细胞肺癌的诊断敏感性为92.54%,特异性为84.19%,其曲线下面积为0.913[95%CI(0.726,1.031)]。结论血清外泌体miR-184在非小细胞肺癌中呈高表达,其临床诊断效能较高,可能成为非小细胞肺癌早期诊断的生物学标志物。
Objective To investigate into the expression level and diagnostic value of serum exosome miR-184 in non-small cell lung cancer. Methods 112 patients with non-small cell lung cancer, 60 with pneumonia and40 healthy subjects were selected as the research objects. Three plasma samples were collected respectively,serum exosome miRNA, QRT-PCR method was used to detect the expression of miR-184 in three groups of exosomes, and the ROC curve was used to evaluate the diagnostic efficacy of exosome miR-184 in non-small cell lung cancer. Results In this study, the purified samples contained a large number of exosomes and had previously reported characteristics of exosomes. The expression of miR-184 in three groups of exosomes was significantly different between each other(P〈0.05). And the expression of miR-184 in the serum of patients with NSCLC was significantly higher than that of patients with pneumonia and healthy subjects(P〈0.05). The ROC curve indicated that the serum exosome miR-184 had no effect on non-small cell lung cancer. The diagnostic sensitivity was 92.54%, the specificity was 84.19%, and the area under the curve was0.913 [95% CI(0.726, 1.031)]. Conclusion Serum exosome miR-184 is highly expressed in non-small cell lung cancer, its clinical diagnostic efficiency is high, and it may become a biomarker for the early diagnosis of non-small cell lung cancer.
