先兆早产阴道微生态失衡状况分析 被引量:10

Analysis of vaginal microecological dysbiosis in threatened premature labor

摘要 目的分析先兆早产妇女阴道微生态失衡状况。方法选择2013-2014年在我科住院的保胎的孕28周~36+6周无明显诱因先兆早产的单胎妊娠期妇女87例。取其阴道分泌物,选用阴道pH值、阴道清洁度、过氧化氢(H2O2)浓度、白细胞酯酶(LE)和唾液酸苷酶(SnaSe)等指标对阴道微生态进行评价。结果先兆早产妇女阴道微生态失衡发生率为70.1%。阴道分泌物检查清洁度Ⅲ~Ⅳ不正常者61例,乳杆菌均阴性。H2O2阳性率为47.5%,H2O2+LE阳性率为13.1%,显微镜检查出线索细胞+LE+H2O2+SnaSe阳性率6.6%,涂片镜检查出滴虫+LE+H2O2阳性率为3.3%,涂片镜检查出酵母样真菌孢子+LE+H2O2阳性率14.8%。结论先兆早产孕妇阴道微生态失衡率较高,可能是导致早产及足月前胎膜早破的重要原因。 Objective To analyse the vaginal microecological dysbiosis in threatened premature labor.Methods 87 pregnant women with 28 to 36+6 weeks of gestation,who had threatened premature labor with no obvious inducement were enrolled.The specimens of their vaginal discharge were collected for the evaluation of vaginal microecology based on vaginal pH,vaginal cleanness,concentration of H2 O2,LE,SnaSe and other indicators.Results The incidence rate of vaginal microecological imbalance in pregnant women with threatened premature labor was 70.1%.There were 61 cases with vaginal cleanness of III^IV and negative result of lactobacillus detection.The incidence rate of positive H2 O2 was 47.5%,while that of positive H2 O2+LE was 13.1%;the rate of positive H2 O2,LE,SnaSe and clue cells was 6.6% by microscopy,that of positive H2 O2,LE and yeast-like fungal spores was 14.8% by smear examination,and that of positive H2 O2,LE and trichomonad was 3.3% by smear examination.Conclusion The incidence rate of vaginal microecological imbalance in pregnant women with threatened premature labor is high,which is possibly an important cause for premature labor and premature rupture of membranes.
