干粉性抗凝剂测定血沉的初步观察

Preliminary observation on ESR determination by dry-powder anticoagulant

摘要 目的初步观察用干粉抗凝剂测定血沉的效果。方法选取两根内置109mmol/L枸橼酸钠0.4 ml的血沉测定管,分别作为标准管和干粉管(80℃烘干后),随机选取31例病人,抽取新鲜血液4.0 ml,分别按随机顺序注入标准管和干粉管,新鲜血量分别为1.6 ml及2.0 ml,混匀后以同样仪器测定血沉值。结果标准管、干粉管血沉值分别为(25.03±24.38)mm/hr,(38.15±33.44)mm/hr,两者比较差异有统计学意义(t=0.041,P<0.05);散点图示干粉管血沉值(Y)与标准管血沉值(X)呈线性正相关,经回归分析两者关系的回归方程为Y=1.31X-5.46(r=0.91,F=279.72,P<0.01)。结论目前使用的标准血沉测定由于血液受到抗凝剂的稀释,与使用干粉性同样抗凝剂相比,其血沉值有一定降低。使用干粉性抗凝剂测定血沉更能真实反映临床实际测定值。 Objective To observe preliminarily the effect of measuring the ESR with dry powder anticoagulant. Methods Two blood sedimentation tubes with 0. 4 ml 109 mmol/L sodium citrate were used as standard tubr and dry powder tube( after drying at80 ℃). 31 patients were selected randomly,and 4. 0 ml of fresh blood was drawn in random order. The fresh blood were injected into the standard tube and the dry powder tube. The volume were 1. 6 ml and 2. 0 ml respectively. After mixing,the blood sedimentation rate was measured with the same instrument. Results The ESR of the standard tube and dry powder tube were( 25. 03± 24. 38) mm/hr and( 38. 15 ± 33. 44) mm/hr respectively; the difference is statistically significant( t = 0. 041,P〈0. 05). The scatterplot shows that the there was a linear positive correlation between the ESR of dry powder tube( Y) and the standard tube( X),and the regression equation for the relationship was Y = 1. 31 X-5. 46( r = 0. 91,F = 279. 72,P〈0. 01). Conclusion Compared with the use of the same anticoagulant,the current standard ESR measuring is certainly reduced because it is diluted by anticoagulant. The use of dry powder anticoagulant to determine ESR can reflect the actual clinical value more effectively.
