绵阳市15～24周岁男男性行为者HIV感染现状及影响因素

Baseline of a cohort study in Mianyang on MSM aged between 15-24 years:HIV infection and influence factors

目的对年轻男男性行为者（YMSM）进行队列研究基线调查,了解其艾滋病病毒（HIV）感染现状及影响因素。方法以四川省绵阳市15～24周岁MSM为对象,用滚雪球抽样法,在活动现场接受干预前进行匿名调查和血清学检测,建立研究队列。分别用χ2检验、非条件Logistic回归分析进行队列基线调查HIV感染影响因素单因素及多因素分析。结果有效问卷362份。362名调查对象近6个月63.8%（230人）发生过肛交,54.3%（125人）每次肛交使用安全套。HIV确证感染率9.1%（33人）,梅毒感染率6.1%（22人）。多因素分析结果,职业、本地居住时间、性角色、抑郁症状、吸食冰毒、近1年接受过安全套发放、寻找性伴场所、近6个月肛交每次使用安全套、梅毒感染等,是YMSM的HIV感染的独立影响因素（P均〈0.05）。结论 YMSM的HIV感染率高,影响因素多样,多种危险行为和心理障碍增大了HIV感染风险,是防治重点。

Objective To explore HIV infection and influence factors on young men who have sex with men（YMSM）as a baseline for a cohort study.Methods The snowball sampling method was used to recruit the YMSM aged 15-24 years in Mianyang of Sichuan province to build a cohort by an anonymous survey and serological testing before intervention.The influence factors of HIV were analyzed with one-factor analysis and multiple factors analysis byχ2 test and unconditional logistic regression analysis.Results There were 362 valid questionnaires collected in total,and 63.8% of them had anal sex,and 54.3 % used condom every time in recent 6 months.The confirmed rate of HIV and syphilis infection was 9.1 % and 6.1%,respectively.Multiple-factors analysis showed that occupations,local residence time,sex role,depression,taking methamphetamine,acceptance of condom in the past year,places to look for sex partner,protected anal sex behavior for the past 6 months,and syphilis were the dependent influence factors of HIV infection among YMSM（P〈0.05）.Conclusion Our study reveals that there is a high HIV infection rate among YMSM,with complex factors.Risk behaviors and psychological obstacles increase the risks of HIV infection,and are the key points for HIV prevention and treatment.
