腰椎术后切口感染病人的病原菌及耐药性分析 被引量:2

PATHOGENIC BACTERIA AND THEIR DRUG RESISTANCE IN PATIENTS UNDERGOING INCISION INFECTION AFTER LUMBAR SPINAL SURGERY

摘要 目的对腰椎退行性疾病手术后切口感染的病人进行细菌学分析,为抗菌药物治疗提供指导。方法对2015年8月—2017年5月我院治疗的36例腰椎术后切口感染病人的病原菌进行常规方法的培养、鉴定,并对其耐药性进行分析。结果革兰阳性球菌感染占61.11%,其中以表皮葡萄球菌最多,占革兰阳性球菌的45.45%。革兰阴性杆菌感染占38.89%,其中以大肠埃希菌最多,占革兰阴性杆菌的57.14%。革兰阳性球菌对万古霉素的敏感性最高。不同种类革兰阴性杆菌的药敏结果存有较大差异。结论腰椎退行性疾病病人术后切口感染治疗的抗生素应用,应该根据细菌培养和药敏结果进行个体化选择。 Objective To perform a bacteriological analysis of patients undergoing incision infection after the surgery for lumbar degenerative disease,and to provide a guidance for the treatment with antibacterial agents. Methods The pathogenic bacteria of 36 patients with incision infection after lumbar spinal surgery in our hospital from August 2015 to May 2017 were cultured and identified using conventional methods,and their drug resistance was analyzed. Results Gram-positive coccus infection accounted for 61.11%,and Staphylococcus epidermidis was the most common Gram-positive coccus,which accounted for 45.45%.Gram-negative bacilli infection accounted for 38.89%,and Escherichia coli was the most common Gram-negative bacillus,which accounted for 57.14%.Gram-positive coccus had the highest sensitivity to vancomycin,and there were significant differences in drug susceptibility results between different types of Gram-negative bacilli. Conclusion The use of antibiotics for postoperative incision infection in patients with lumbar degenerative disease should be individualized according to the results of bacterial culture and drug susceptibility testing.
