平喘方联合清热散穴位敷贴治疗轻度小儿热性哮喘临床观察

Clinical observation on Pingchuan decoction combined with acupoint application with Qingre powder on mild childhood heat asthma

目的观察平喘方联合清热散穴位敷贴治疗轻度小儿热性哮喘的临床疗效及对中医证候评分、咳喘症状、肺部体征、嗜酸性粒细胞(EOS)比率的影响。方法将120例轻度热性哮喘患儿随机分为治疗组60例和对照组60例,对照组予以普米克令舒、博利康尼、异丙托溴铵雾化吸入,每日2次;治疗组予以平喘方口服,并以清热散敷于天突、肺俞穴。2组疗程均为7 d。比较2组咳嗽、喘息及肺部哮鸣音消失时间,评价2组临床疗效,观察2组治疗前后中医证候积分及EOS比率变化。结果治疗组痊愈率和显效率均明显高于对照组(P均<0.05)。治疗后,2组咳嗽、喘息症状及肺部哮鸣音消失时间比较差异均无统计学意义(P均>0.05);2组中医证候积分均明显降低(P均<0.05),且治疗组次要证候积分和总积分均明显低于对照组(P均<0.05);2组EOS比率均较治疗前显著下降(P均<0.05),2组间比较差异无统计学意义(P均>0.05)。结论平喘方联合清热散穴位敷贴合离子导入技术治疗轻度小儿热性哮喘能够改善中医证候,缩短疗程,调节免疫功能,在主要症状体征改善方面与西医常规治疗效果相当。

Objective It is to observe the clinical curative effect of Pingchuan decoction combined with acupoint application with Qingre powder in the treatment of mild infantile febrile asthma and effect on TCM syndrome score,disappeared time of symptom and sign,Eosinophils count（ EOS）. Methods 120 patients were randomly divided into treatment group（ 60 cases） and control group（ 60 cases）,the control group was treated with Budesonide,Terbutaline and Ipratropium bromide inhalation,2 times a day,the treatment group was treated with Pingchuan decoction orally,and acupoint application with Qingre powder on Tiantu and Feishu point. Both groups were treated for 7 days. The changes of syndrome scores of TCM and EOS before and after treatment were observed,the disappeared time of cough,asthma and lung wheezing were compared and the clinical curative effects were evaluated in both groups. Results The total effective rate of the treatment group was higher than that in the control group（ P〈0. 05）. After treatment,the differences in the disappeared time of cough,asthma and lung wheezing were not statistically significant between the two groups（ P〈0. 05）. After treatment,TCM syndrome scores were improved significantly in both groups（ P〈0. 05）,and the improvements in the treatment group were better（ P〈0. 05）,the EOS of the two groups were decreased significantly compared with before treatment（ P〈0. 05）,there was no significant difference between the two groups（ P〉0. 05）. Conclusion Pingchuan decoction combined with acupoint application with Qingre powder can improve the TCM syndrome,shorten the course of treatment,regulate the immune in the treatment of mild infantile febrile asthma,its curative effect is similar to western medicine in the improvement of main symptoms and signs.
