加味阳和汤联合三苯氧胺对浆细胞性乳腺炎患者炎性因子及免疫功能的影响

Effect of Jiawei Yanghe decoction combined with tamoxifen on the immune function and inflammatory factors in patients with plasma cell mastitis

目的分析探讨加味阳和汤联合三苯氧胺对浆细胞性乳腺炎(PCM)患者炎性因子及免疫功能的影响。方法将90例PCM患者随机分为2组,其中观察组48例予以加味阳和汤联合三苯氧胺治疗,对照组42例予以三苯氧胺治疗,分别于治疗前和治疗3个月后检测2组患者血浆炎症因子IL-1α、IL-1β、IL-6、IL-8、TNF-α水平和血清免疫球蛋白Ig G、Ig M、Ig A及补体C3、C4水平,评定2组临床疗效。结果治疗后观察组有效率明显高于对照组(P<0.05)。治疗前2组各炎症因子水平和免疫指标比较差异均无统计学意义(P均>0.05);治疗后,2组各炎症因子水平均较治疗前明显降低(P均<0.05),且观察组降低程度更明显(P均<0.05);2组免疫指标Ig A、Ig G、Ig M、C3、C4均显著改善(P均<0.05),观察组改善情况更显著(P均<0.05)。结论加味阳和汤联合三苯氧胺治疗PCM能够降低患者炎症因子水平,减轻炎症反应,改善患者免疫功能,效果良好,值得在临床上推广应用。

Objective It is to investigate the effect of Jiawei Yanghe decoction combined with tamoxifen on the immune function and inflammatory factors in patients with plasma cell mastitis（ PCM）. Methods 90 patients with PCM were randomly divided the observation group（ 48 cases） and the control group（ 42 cases）. The patients in the observation group were treated with Jiawei Yanghe decoction combined with tamoxifen while the patients in the control group were treated with tamoxifen. The changes of the levels of inflammatory factors IL-1 alpha,IL-1 beta,IL-6,IL-8,TNF-α and serum immunoglobulin Ig G,Ig M,Ig A in serum and levels of complement C3 and C4 were observed before treatment and after treatment for3 months in both groups,the clinical curative effects between the two groups were compared. Results The effective rate of the observation group was higher than that in the control group（ P〈0. 05）. Before treatment,there was no significant difference between the 2 groups in the levels of inflammatory factors and immune indexes（ P〉0. 05）. After treatment,the inflammatory factors in the two groups of were lower than those before treatment,and they were decreased more in the observation group（ P〈0. 05）. The immune indexes of Ig A,Ig G,Ig M,C3 and C4 were significantly decreased in the two groups,and the improvements of the observation group were more significant（ P〈0. 05）. Conclusion Modified Yanghe decoction combined with tamoxifen is suitable for the treatment of PCM,which can decrease the level of inflammatory factors,reduce inflammation and improve the immune function of patients. This method is worthy of clinical application.
