脑出血急性期血清前清蛋白水平与患者预后的相关性分析

Correlation analysis of serum prealbumin level and prognosis of patients with acute cerebral hemorrhage

目的探究脑出血急性期血清前清蛋白(PA)与患者预后的相关性;方法采用回顾性研究方法,以2012年1月至2017年6月在该院住院治疗的200例急性脑出血患者作为脑出血组;另选取同期健康体检者200例作为对照组,比较血清PA水平,在发病3个月后采用改良RanRin量表对脑出血患者神经功能进行评估,分为预后良好组(0~2分)和预后不良组(>2~6分);将相关指标进行多因素Logistic回归分析。结果脑出血组血清中清蛋白水平明显高于对照组,差异有统计学意义(t=7.821,P<0.05);不同预后组中年龄、低密度脂蛋白、尿酸、PA、纤维蛋白原和D-二聚体水平差异有统计学意义(t=6.321、4.312、4.525、4.124、4.672、3.073,P<0.05);年龄、脑出血急性期PA可以作为脑出血后的独立预测指标。结论急性脑出血期低PA水平患者预后较差,该指标在脑出血急性期中异常可作为患者预后结局的预测指标。

Objective To explore the correlation between the prognosis and the serum prealbumin（PA）of patients with acute cerebral hemorrhage.Methods This study was a retrospective study,a total of 200 acute cerebral hemorrhage patients in Shanghai Smelter Hospitalas from January 2012 to June 2017 were recruited into cerebral hemorrhage group,and 200 healthy persons as the control group.Compared the content of PA,the modified Rankin scale were used to evaluate the neurological function of patients with cerebral hemorrhage after the onset of three months,divided into good prognosis group（0-2）and poor prognosis group（〉2-6）.Logistic regression analysis the relevant indicators of many factors.Results The content of PA in cerebral hemorrhage group was significant higher than that of control group,there was significant difference（t=7.821,P〈0.05）.There were significant difference on low density lipoprotein,fibrinogen,uric acid,PA and Ddimer in different prognosis groups（t=6.321,4.312,4.525,4.124,4.672 and 3.073,P〈0.05）.Age and PA after acute cerebral hemorrhage could be used as independent predictors of cerebral hemorrhage.ConclusionAcute cerebral hemorrhage with low PA has poor prognosis,the index in the acute phase of the anomaly could predicte the prognosis of patients with cerebral hemorrhage.
