盆底肌锻炼联合膀胱训练对前列腺电切术后暂时性尿失禁的影响

Effect of pelvic floor muscle exercise combined with bladder training on temporary incontinence after transurethral resection of prostate

目的探讨盆底肌锻炼联合膀胱训练对前列腺电切患者术后暂时性尿失禁的影响。方法选取2014年1月至2016年12月我院收治的良性前列腺增生患者184例,根据随机数字表法将其分为观察组(93例)和对照组(91例),两组患者均在连续硬膜外麻醉下行经尿道前列腺电切术。对照组术前、术后均给予常规护理与膀胱训练。观察组在常规护理的基础上自术前5 d至术后尿管拔除后给予盆底肌锻炼联合膀胱功能训练。比较两组患者拔管后48 h内暂时性尿失禁发生情况。结果观察组拔管后48 h内暂时性尿失禁发生率低于对照组(P<0.05);观察组ICIQ-SF评分和尿失禁持续时间均低于对照组(P<0.05)。结论盆底肌锻炼联合膀胱训练可降低前列腺电切术后暂时性尿失禁的发生率及程度,缩短其持续时间。

Objective To explore the effect of pelvic floor muscle exercise combined with bladder training on temporary incontinence after transurethral resection of prostate. Methods A total of 184 patients with benign prostatic hyperplasia in our hospital from January 2014 to December 2016 were divided into observation group （93 cases） and control group （91 cases） according to the random number table. Patients of both groups underwent continuous epidural anesthesia and transurethral resection of the prostate. The control group was given routine nursing and the bladder training before and after operation. On the basis of routine nursing, the observation group was treated with pelvic floor muscle exercise combined with bladder training at 5 d before operation and after removal of catheter. The occurrence of temporary incontinence within 48 h after extubation in the two groups of patients were compared. Results The incidence of temporary incontinence in the observation group was lower than that of the control group （P〈0.05）. The ICIQ-SF score and the duration time of urinary incontinence in the observation group were lower than those in the control group （P〈0.05）. Conclusion Pelvic floor muscle exercise combined with bladder training can reduce the incidence and extent of transient incontinence and shorten the duration time in patients with transurethral resection of prostate.
