测量三血管切面肺动脉及主动脉内径诊断胎儿心脏大动脉异常 被引量:11

Measurement of the Inner Diameter of the Pulmonary Artery and Aorta on Three Vessels View to Diagnose Fetal Cardiac Great Arteries Abnormalities

摘要 目的探讨三血管切面肺动脉(Pulmonary Artery,PA)、主动脉(Aorta,AO)内径及二者比值(PA/AO)对胎儿心脏大血管异常的诊断价值。方法选取正常孕妇569例和三血管切面PA或AO内径异常孕妇52例,在三血管切面上测量AO及PA内径,并计算PA/AO。结果正常胎儿PA、AO内径与孕周呈正向关性;PA内径大于AO内径,差异有统计学意义(P<0.05);PA/AO与孕周无明显相关性,二者比值固定在1.0~1.2之间。以PA/AO在1.0~1.2之间作为参考范围,异常组中三血管切面上PA、AO内径及二者比值存在异常的胎儿检出率为90.4%。结论测量三血管切面肺动脉及主动脉内径可成为胎儿先心病筛查的一个重要指标。 Objective To explore the diagnostic value of three vessels view of the inner diameter of pulmonary artery(PA) and aorta(AO) and the ratio of PA/AO on fetal cardiac great arteries abnormalities. Methods 569 normal pregnant women and 52 cases of pregnant women with abnormal PA or AO inner diameter on three vessels view were selected. AO and PA inner diameter were measured on three vessels view and the ratio of PA/AO was calculated. Results The PA, AO inner diameter in normal fetal were positively correlated with their gestational age. The inner diameter of PA was greater than that of AO(P〈0.05). There was no signifi cant correlation between the ratio of PA/AO and gestational age, and the ratio was between 1.0 and 1.2. Take the PA/AO ratio between 1.0 and 1.2 as reference range, the detection rate of abnormalities of the PA, AO inner diameter and the ratio of PA/AO on three vessels view in the abnormal group was 90.4%. Conclusion Measurement of the inner diameter of the pulmonary artery and aorta on three vessels view can be an important indicator for the screening of fetal congenital heart disease.
