摘要
目的探讨糖尿病前期合并肥胖患者血脂水平与胰岛素抵抗的关系。方法回顾性选取2016年1~6月本院收治的糖尿病前期患者80例,依据体质量指数(BMI)分为肥胖组(n=48例)和非胖组(n=32例),所有患者均检测空腹血糖(FPG)、餐后2h血糖(2h PG)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMAIR)、血清三酰甘油(TG)、胆固醇(TC)、高密度脂蛋白(HDL-C)、低密度脂蛋白(LDL-C),分析患者血脂水平与胰岛素抵抗的关系。结果肥胖组BMI、FINS、HOMA-IR、TG、LDL-C明显高于非胖组,差异有统计学意义(P<0.05),肥胖组和非胖组FPG、2h PG、TC、HDL-C基本相同,差异无统计学意义(P>0.05);Pearson相关性分析结果显示,TG、LDL-C分别与BMI、FINS、HOMA-IR呈正相关(P<0.05)。结论糖尿病前期合并肥胖患者血脂代谢紊乱与胰岛素抵抗密切相关,提示控制血脂水平可能有助于抑制或逆转糖尿病前期的病情。
Objective To explore the relationship between blood lipid levels and insulin resistance in patients with pre-diabetes complicated with obesity. Methods Eighty patients with prediabetes admitted to our hospital from January to June 2016 were retrospectively selected,and divided into obese group(n=48 cases) and non-obese group(n=32 cases) according to body mass index(BMI).Fasting blood glucose(FPG),2 h postprandial blood glucose(2 h PG),fasting insulin(FINS),insulin resistance index(HOMA-IR),serum triglyceride(TG),cholesterol(TC),high density lipoprotein(HDL-C) and low density lipoprotein(LDL-C) were detected in all patients.The relationship between blood lipid level and insulin resistance was analyzed. Results BMI,FINS,HOMA-IR,TG and LDL-C in obese group were significantly higher than those in non obese group(P〈0.05).FPG,2 h PG,TC and HDL-C in obese group and non obese group were basically the same,and the difference was not statistically significant(P〈0.05).Pearson correlation analysis showed that TG and LDL-C were positively correlated with BMI,FINS,and HOMA-IR,respectively(P〈0.05). Conclusion The disorder of blood lipid metabolism is closely related to insulin resistance in prediabetic patients with obesity,suggesting that the level of blood lipid control may help to inhibit or reverse the condition of prediabetes.
