摘要 目的探讨Mtwo机用镍钛锉行磨牙根管预备和传统不锈钢K锉行磨牙根管预备的优劣差异。方法选择2015年8月~2017年8月我科需要进行磨牙根管治疗的患者96例(100牙)为研究对象,按照随机数字表法分为实验组与对照组,每组各50牙。其中实验组采用Mtwo机用镍钛锉行磨牙根管预备,对照组采用传统不锈钢K锉行磨牙根管预备。观察比较两组患者预备术后疼痛程度评分情况、根管充填质量以及根管预备时间。结果实验组患者术后疼痛发生率为4.00%,显著低于对照组的22.00%(P<0.05)。实验组和对照组根管恰填分别有46颗(92.00%)和35颗(70.00%),实验组根管恰填率明显高于对照组(P<0.05)。其中实验组中有2颗欠填,2颗有少量根充糊剂超出根尖孔,无根管偏移情况,根管形态均为根方小、冠方大的连续锥形,锥度和流畅度好;对照组有9颗欠填2~4mm,6颗有少量根充糊剂超出根尖孔,3颗根管走向改变,有台阶形成,剩余根管形态均为根方小、冠方大的连续锥形,锥度和流畅度较好。实验组根管预备时间为(7.38±0.82)min,明显少于对照组的(12.84±1.61)min,差异有统计学意义(P<0.05)。结论临床上采用Mtwo机用镍钛锉行磨牙根管预备较传统不锈钢K锉减轻患者术后疼痛程度、提高根管充填质量且节省根管预备时间,值得临床上推广使用。 Objective To investigate the difference of advantages and disadvantages of Mtwo nickel titanium file for molar root canal preparation and traditional stainless steel K file for molar root canal preparation. Methods Ninety-six patients(100 teeth) who needed molar root canal therapy were selected as subjects from August 2015 to August 2017. They were equally divided into the experimental group and the control group according to the random number table method, with 50 teeth in each group. The experimental group was treated with Mtwo nickel titanium file for molar root canal preparation, while the control group was treated with traditional stainless steel K file for molar root canal preparation. The pain score, the quality of root canal filling and the time of root canal preparation were compared between the two groups. Results The incidence of postoperative pain in the experimental group was 4%, which was significantly lower than 22% in the control group(P〈0.05). There were 46(92%) and 35(70%) root canals in the experimental group and the control group respectively, and the filling rate of the root canal in the experimental group was significantly higher than that in the control group(P〈0.05). In the experimental group, 2 teeth were under filling, and the 2 teeth had a small amount of root filling agent beyond the apical hole and no root canal migration. The root canal morphology was a small root square, a continuous cone with a large crown, and good taper and fluency. In the control group, 9 teeth were under 2-4 mm, the 6 teeth had a small amount of root filling agent beyond the apical hole, the 3 root canals were changed, and the steps formed. The remaining root canal forms were the small root square, the continuous cone of the crown square, and the taper and fluency better. The root canal preparation time in the experimental group was(7.38±0.82) min, which was significantly less than that in the control group(12.84±1.61) min, and the difference was statistically significant(P〈0.05). Conclusion The clinical use of Mtwo nickel titanium file for molar root canal preparation is better than the traditional stainless steel K file to reduce the degree of postoperative pain, improve the quality of root canal filling and save the time of root canal preparation. It is worth popularizing in clinical practice.
