robust优化在乳腺癌调强放疗中的应用

摘要 目的探讨robust优化在乳腺癌调强放疗中的应用。方法选取10例左侧乳腺癌改良根治术后患者为研究对象,应用Raystation计划系统设计划分为A组与B组,A组为加入robust优化的调强适形放射治疗(IMRT)计划、容积旋转调强放射治疗(VMAT)计划,B组为不加入robust优化的IMRT计划、VMAT计划,剂量体积约束条件及处方剂量相同。比较A组与B组适形指数(CI)及均匀指数(HI)、靶区和危及器官剂量。结果 A组计划在射野中心发生位移时,能获得较满意的剂量分布。与B组比较,A组计划靶区(PTV)的CI和临床靶区(CTV)的处方剂量覆盖率明显提高,PTV的HI明显降低,差异均有统计学意义(均P<0.05);而危及器官的剂量比较,差异基本无统计学意义(均P>0.05)。结论对于乳腺癌改良根治术后的患者,用Raystation计划系统设计IMRT计划和VMAT计划时可加入robust优化,以更好地提高靶区的剂量覆盖率。
