参芪地黄汤联合缬沙坦治疗早期糖尿病肾病(气阴两虚型)临床观察

Clinical observation on Shenqi Dihuang Decoction combined with Valsartan treating early diabetic nephropathy(syndrome of deficiency of both qi and yin)

目的:探讨参芪地黄汤联合缬沙坦治疗早期糖尿病肾病(气阴两虚型)的临床效果。方法:将72例早期糖尿病肾病(气阴两虚型)患者根据随机数表法分为两组,各36例。对照组采用单纯缬沙坦治疗,观察组予参芪地黄汤联合缬沙坦治疗,比较两组实验室指标[尿素氮(BUN)、血肌酐(SCr)、尿白蛋白排泄率(UAER)、空腹血糖(FPG)、餐后2h血糖(2hPG)]、中医证候积分、尿微量白蛋白(mALB)、尿N-乙酰-β-D-葡萄糖苷酶(NAG)水平及并发症情况。结果:观察组治疗8周时实验室指标水平、中医证候积分、尿m ALB及尿NAG水平均低于对照组(P<0.05,P<0.01);两组治疗期间不良反应发生率对比,差异无统计学意义。结论:给予早期糖尿病肾病(气阴两虚型)患者参芪地黄汤联合缬沙坦治疗疗效确切、安全性较好。

Objective： To investigate the clinical effect of Shenqi Dihuang Decoction combined with valsartan treating early diabetic nephropathy（EDN）（syndrome of deficiency of both qi and yin）. Methods： A total of 72 EDN patients（syndrome of deficiency of both qi and yin） were divided into two groups by random number table, with 36 cases in each group. The control group was given Valsartan, while the observation group was treated with Shenqi Dihuang Decoction combined with Valsartan. The laboratory indexes, TCM symptom score, urine microalbumin（m ALB）, urine N-acetyl-β-D-glucosidase（NAG） and complications were compared between the two groups. Results： After treatment for 8 weeks, the levels of blood urea nitrogen（BUN）, serum creatinine（SCr）, urinary albumin excretion rate（UAER）, fasting plasma glucose（FPG）, 2 hours postprandial blood glucose（2 h PG）, TCM symptom score, urine m ALB and urine NAG levels in the observation group were lower than those in the control group（P〈0.05, P〈0.01）; there was no statistical difference in the incidence of adverse reactions between the two groups during treatment. Conclusion： Shenqi Dihuang Decoction combined with Valsartan in the treatment of EDN（syndrome of deficiency of both qi and yin） is effective, and has better security.
