骨密度与全膝关节置换术后早期疼痛的相关性研究 被引量:1

Study of Correlation between Bone Mineral Density and Early Pain after Total Knee Arthroplasty

摘要 目的:观察初次人工全膝关节置换术(total knee arthroplasty,TKA)后不同骨量患者的术区疼痛VAS评分、术后自控镇痛(Patient-controlled analgesia,PCA)药物使用量与骨密度(bone mineral density,BMD)的相关性。方法:前瞻性观察2014年6月至2016年5月间行单侧初次TKA的女性患者共计54例,年龄61~79岁,平均68.7岁,术前诊断均为膝关节骨性关节炎;记录并分析TKA术后48 h时的疼痛视觉模拟评分(visual analogue scale,VAS)、术后48 h吗啡消耗量、年龄以及体质指数(body mass index,BMI)与骨密度的相关性。结果:术后48 h的VAS评分及吗啡消耗量均与骨密度明显相关。结论:骨密度越低的患者,TKA术后疼痛越严重,对镇痛药物的使用量越大。 Objective To investigate the correlation between bone mineral density( BMD) and the index of visual analogue scale( VAS) and patient-cotrolled analgesia( PCA) with various bone mass density after total knee arthroplasty( TKA).Methods A total of 54 female patients with unilateral primary TKA between June 2014 and May 2016 were prospectively observed,ranging from 61 to 79 years of age,with an average age of 68.7 years. Preoperative diagnosis was knee osteoarthritis. The correlations between visual analogue scale( VAS),morphine consumption in 48 hours,age,body mass index( BMI) and bone mineral density at 48 hours after TKA were analyzed. Results There was significant correlation between BMD and index of VAS and total analgesic consumption( P〈0.05). Conclusion Lower BMD leads to more severe pain and more consumption of the analgesic drugs.
